Home Blood Pressure Monitoring Before and After COVID-19 Vaccination in Patients at High Risk of...

The coronavirus SARS-CoV-2 causing COVID-19 was pandemic since 2020. Vaccination is one of the most important measure to control the outbreak. The countries of the world started the vaccination since December, 2020. Taiwan started since March, 21st, 2021. Currently, Taiwanese people receive COVID-19 vaccination by either AZ or Moderna; Pfizer/BNT vaccine may import in the near future. The side effects post vaccination including injection site pain, fatigue, headache, myalgia, chills, arthralgia, fever, and so on. Incidence of side effects more frequent in young people than elders; more frequent in the first dose than second dose in AZ vaccine; more frequent in the second dose in Moderna or Pfizer/BNT vaccine. Taiwan V-watch is a system monitored by CDC for people proactive notification of side effects post COVID-19 vaccination. But it doesn't include the objective cardiovascular parameters, for example, heart rate, arrhythmia, blood pressure, central body temperature (ear temperature) and blood oxygen saturation level. This study aims to monitor the physiologic and psychologic effects, and side effects before and after COVID-19 vaccination in patients at high cardiovascular risk, including hypertension, hyperlipidemia, and type 2 diabetes, and arrhythmia or coronary artery disease. The investigators arrange two weeks of continuous home BP/HR monitoring (7 days pre- and post-vaccination) for evaluation of physiologic effects post vaccination. Due to fever noted in some people post-vaccination, the investigators also measure ear temperature, pulse oximetry, and environmental temperature and humidity, for the reference of BP monitoring. In conclusion, this study may provide important information of cardiovascular response and psychosocial effects after SARS-CoV-2 vaccination for proactive prevention of cardiovascular complications.

Oral Isotretinoin and Covid-19 Infection

There is a theoretical risk that the nasal mucosal dryness caused by oral retinoid medications may increase the risk of patients becoming infected with Covid-19. Isotretinoin is a drug used commonly in dermatology, usually for management of acne and occasionally for management of other dermatological diseases. The most common side effect of oral isotretinoin is mucocutaneous dryness, which can lead to nasal dryness and crusting. Isotretinoin is derived from vitamin A and produces this side effect by arresting the cell cycle of mucus secreting cells in the nasal cavity. The mucus usually secreted moisturises the nasal cavity and provides a protective, lubricating layer overlying the cilia. Cilia and nasal mucous are both believed to have roles in defence against infection and immunity. Hence it was postulated at the start of the pandemic that the effect this medication has on the nasal environment may increase the likelihood of contracting a disease spread by droplet or aerosol particles. In March 2020 we established a departmental Excel database of our patients on oral retinoid medications in order to track those currently receiving treatment. The database includes people taking isotretinoin, alitretinoin and acitretin. The greatest number of patients in this database are in the isotretinoin group and patients taking isotretinoin tend to be a relatively young and fit population. Acitretin on the other hand is often used in an older population who may have other underlying disease comorbidities. There were only a very small number of patients taking alitretinoin in the database. In order to limit confounding variables and provide as clear a result as possible, it was decided to limit this study to comparing the group of patients taking isotretinoin, aged 16-40years, with an age-matched background population from NHS Grampian. This study would examine the electronic patient record of patients aged 16-40years, taking isotretinoin between March and October 2020 and compare their rates of Covid-19 infection, hospitalisation and complications with the rates of the local age-matched background population. Persons taking isotretinoin in the time period would be excluded from the background population. As Covid-19 is a new disease, the existing research literature on this specific topic is extremely limited, and so far this study would be the first in this area.

The investigators gather the effects of people who taking NRICM101 using the Real-World Big Data Study and the network feedback information collection model. This study can be used as a reference for global prevention and control of coronavirus disease 2019 (COVID-19).

Introduction: Coronavirus Disease 2019 (COVID-19) has caused a global pandemic. Epidemiological and clinical inter-individual differences, symptomatology, recovery and humoral response against Severe Acute Respiratory Syndrome Coronavirus 2 (SARS-CoV-2) are key factors to better understand and predict the course of the pandemic. As Health Care Workers (HCWs) are caring for infected patients they are more susceptible to infection, which not only is critical for their own health but also because it results in a shortage of HCWs that seriously affects health services. Thus, maintaining the health and welfare of HCWs and enabling their rapid return to work is vital to overcome this crisis. The ProHEpiC-19 cohort presents data on the immune response of HCWs infected with SARS-CoV-2. This dynamic cohort was started in March 2020 and still continues including participants.

COVID19 Vaccination: Clinical, Laboratory and Cellular Monitoring

Primary objective: 1. Monitor adverse events (Adverse Event Following Immunization - AEFI) in vaccinated subjects and analyze the associations with: (a) demographic variables and anthropometric parameters (sex, age, BMI, etc.); (b) socioeconomic-cultural variables; (c) co-morbidities. Secondary objectives: Monitor the antibody response in vaccinated subjects and analyze the associations with: (a) demographic variables and anthropometric parameters (sex, age, BMI, etc.); (b) socioeconomic-cultural variables; (c) co-morbidities. Assess the quantity (antibody titer) and quality of the anti-Spike immune response induced by vaccination. Evaluate the dynamics of the infection and the protective efficacy of the vaccine against infection / disease by studying the incidence and titre of antibodies against Nucleoprotein (N) induced by natural infection; Evaluate the neutralizing capacity of sera vs. a panel of sera selected and analyzed in house and in in vitro microneutralization systems with live virus; To evaluate the cell-mediated immune response (CMI) to vaccination against SARS-CoV-2 in a subgroup of vaccinees. Methodology: Administration at baseline of a questionnaire for the collection of clinical data. Perform a blood sample to measure antibody response in vaccinated subjects Administer a questionnaire to evaluate adverse events after vaccination Reassess the antibody response 1 month after complete vaccination and 6 and 12 months after the start of vaccination An antibody assay will be drawn in a subgroup before subjecting the subject to the vaccine. In a subgroup, the blood collected will also be collected in a heparinized tube for the study of cell-mediated immunity. In the event that a subject participating in the study presents during the course of the observation the positivity for the molecular research of SARS-CoV-2 RNA to the oro-nasopharyngeal swab, a new sample will be performed and sent to the Istituto Superiore di Sanità for the search for viral variants.
