Arteriosclerosis is a common chronic disease with well known risk factors like diabetes mellitus, hypertension, dyslipidemia, adipositas and smoking, leading to severe complications like myocardial infarction, stroke or peripheral arterial occlusive disease. In addition to life-style factors there is also a genetic predisposition to develop complicated atherosclerosis. Objective: Improve individual risk prediction by clinical phenotyping and genotyping.

The goals of this trial are: To compare the sensitivity and specificity of various tools including Doppler and magnetic resonance imaging (MRI) for diagnosing vertebral artery stenosis and carotid artery stenosis. The positron emission tomography (PET)/MRI fusion imaging will be a promising tool to provide both anatomical and functional imaging for vulnerable plaques. To identify patients at high risk of plaque rupture by image studies, biomedical profile, and gene expression of circulating monocytes.

Our preliminary data suggests that pharmacological inhibition of the mitochondrial fission protein, Drp1, reduced atherosclerotic plaque volume and attenuated macrophage accumulation within the plaque in an ApoE-/- mouse model of wire-induced carotid arterial injury. Furthermore, we hypothesize that modulation of mitochondrial morphology and metabolism with Drp1 inhibition prevents atherosclerosis by reducing monocyte activation and migration. In this research proposal, our overall objective will be to investigate the role of Drp1 in human monocytes and macrophages as novel therapeutic targets for preventing atherosclerosis.
