NEOZOL_Follow-up Study

Locally Advanced Breast Cancer

Breast cancer is the most common cancer in women and is the leading cause of death from cancer in France. The main types of treatment used to treat breast cancer are surgery, chemotherapy, radiotherapy, hormone therapy and targeted therapies. In locally advanced breast cancer, a neo-adjuvant chemotherapy treatment is currently recommended as it may provide a sufficient tumour response to allow for conservative breast treatment. Zoledronic acid (ZOL) is an N-biphosphonate used in the prevention of bone complications and in the treatment of hypercalcaemia In vitro and in vivo studies have shown an effect of ZOL on tumour and endothelial cells, supporting an anti-tumour activity of N-biphosphonates. Between April 2010 and October 2013, 50 patients with locally advanced breast cancer were randomized in the NEOZOL study to receive neoadjuvant chemotherapy with (n=26) or without (n=24) zoledronic acid (ClinicalTrials.gov Identifier: NCT01367288). The primary objective of this study was to evaluate the effect of the addition of ZOMETA® to neoadjuvant chemotherapy on the change in serum VEGF concentration before and after neoadjuvant treatment (i.e. at the time of surgery). Assessment of long-term survival was not planned in the NEOZOL study protocol. The objective of NEOZOL_FU study is to evaluate the survival of patients included in the NEOZOL study, 7 years after inclusion of the last patient. This study will provide essential data on the long-term survival and risk of relapse of patients with locally advanced breast cancer who have received neo-adjuvant chemotherapy with or without zoledronic acid (ZOMETA®).

With an estimated > 2 million women with undetected breast cancer in the United States, the need for improved early detection is imperative. Early diagnosis for these women is key to minimizing quality life-years lost to disease and optimizing success of treatment. Evidence now exists supporting the finding that systemic spread is an early event in the natural history of breast cancer, manifested as a release of single cancer cells from the incident, clinically undetectable tumor, which circulate through the bloodstream and deposit within remote tissues. Reliable and accurate detection of these circulating tumor cells (CTCs) is now possible with a simple peripheral venous blood draw. This study hypothesizes that women with CTCs and no other signs of malignancy have clinically undetectable disease. This study will attempt to validate this technology as a breast cancer screening test and acquire data to determine the clinical validity and utility of this proposed screening methodology on a relatively young, ethnically diverse population who are eligible military health care beneficiaries. Furthermore, this study will attempt to bank identified CTCs in order to perform additional molecular analyses in the future. The specific aims are to develop a simple, reliable, cost-effective, and clinically relevant breast cancer screening test in order to identify subclinical disease early in its natural history in subjects at risk of progression to clinically apparent disease over the ensuing decade. The ultimate goal is to decrease the treatment-related morbidity and cause-specific mortality of breast cancer. An experienced team devoted to the care of patients with breast disease has been assembled to achieve this goal.

Over the past five years, a spectrum of studies has shown that the stromal cell populations surrounding or embedding cancer cells in a variety of different tumor types are intrinsically involved in the mechanisms affecting tumor growth and metastasis. Among these cells, fibroblasts and myeloid derived suppressor cells (MDSCs) have been shown to play key roles in the regulation of tumor cell behaviors, via differential gene expression, regulation and secretion of specific chemokines (e.g., RANTES, CXCL12 and CXCL14), cytokines (e.g., TNF-α, IL-1β, NF-κB) and control by inflammatory and immunomodulatory cells, molecules or potent growth factors. Of particular interest is that the various stromal immune cells (dendritic cells and T cells) and inflammatory MDSCs found in primary and metastatic tumors behave very differently from those present in normal or non-cancerous counterpart tissues. In light of these "new understandings" in cancer biology, the investigators believe that systematic and innovative approaches to further research are very much needed in Taiwan and at our Academy. Here the investigators propose to investigate the tumor suppressive effect of anti-inflammatory phytomedicines on regulation of stromal immune cells and fibroblasts in breast cancers. The investigators will employ the TS/A and 4T1 mouse mammary carcinoma system to conduct an in vivo study of several specific candidate phytomedicines (cytopiloyne, Wedelia chinensis, shikonin, emodin and others that the investigators have already identified as conferring anti-inflammation related activities) for inhibition of TS/A and 4T1 tumor growth and specific effects on inflammation-associated, cellular and molecular functions of stromal immune cells, fibroblasts and MDSCs. When potent and specific anti-tumor effects are detected, the investigators will extend our study to a three-dimensional collagen/Matrigel culture system for ex-vivo study focusing on the stromal cell-mediated or -associated anti-tumor effects of TS/A or 4T1 cells, using the "organoid" tissue culture systems the investigators have previously developed in our laboratory (JNCI, 1979; Cancer Res., 1981). The investigators will establish reconstituted TS/A or 4T1 tumor cells with the stromal immune cells in a co-cultivation system. When the investigators have achieved demonstrable success, the investigators will extend the mouse system to human breast cancers (including primary and metastatic tumors) by a close collaboration (which the investigators have already established) with Drs.

You are invited to participate in a research study looking at metabolism (breakdown) and effects of aromatase inhibitors. The purpose of this research is to try to identify which women who take an aromatase inhibitor are more likely to have certain benefits or side effects from the drug. We will do so by determining whether there are differences that normally occur in genes that you have inherited from your parents that might influence the way individuals respond to medications.
