Potentially Inappropriate Prescription Associated to Multimorbidity

A prospective multicenter cohort study in 5 hospitals in Spain will be initiated in 2016. Objectives: To estimate and describe patterns of multimorbidity and polypharmacy in patients over 64 admitted for a chronic disease exacerbation. To analyze the potentially inappropriate prescribing (PPI) drugs according to STOPP / START (SS) criteria. To evaluate the relationship between multimorbidity and PPI and adverse medication (RAM) preventable reaction. The cohort will include 800 patients >64 years admitted in internal medicine and / or geriatric department of 5 hospitals of the National Health Service. Application of the SS criteria (released in 2015) on admission and at discharge, and collection of demographic and clinical variables including comorbidities, baseline chronic medication, geriatric syndromes, functional capacity and RAM. Descriptive analysis and bivariate parametric or nonparametric tests will be applied to analyze relationship between morbidity, polypharmacy, SS criteria and RAM. Intraobserver agreement will be assessed for SS criteria. Multiple regression techniques will be applied, where the dependent variable will be the PPI or the RAM.

Characteristics and Clinical Implications of a Clinical Decision Support System

The investigators propose to study the feasibility, acceptability, usability and outcomes of a new clinical decision support system for clinicians of opioid therapy in the context of specialist palliative care for serious chronic illness. The system provides clinicians with patient-specific genetic information on opioid responsiveness and multi-drug interactions. This clinical decision support system should improve the clinician's ability to identify the optimal pain medication and dosage, and reduce risks associated with multi-drug treatment. Investigators will conduct clinician survey's to collection information about the clinical decision support system. Investigators will also conduct patient related questionnaires to determine any benefits or improvements in quality of life and symptom management from the clinical decision support system.

Evaluation of 3D Imaging Ulcer Camera

The aim of the study is to evaluate the intra- and inter-rater variability of wound measurements using the 3D camera and to compare with standard measurement methods (2D image method and gel injection). Forty-eight patients with wounds of various sizes are measured by four clinicians. Each wound is measured twice with the 3D camera, once by 2D image method and once by gel injection into wound cavity by two clinicians.

The investigators will conduct an innovating study combining quantitative and qualitative variables in a randomized controlled trial to confirm our hypothesis.

Implementing Advance Care Planning Conversation Tools in Family Practice

This study evaluates the use of advance care planning conversation tools with patients attending their family doctor's office. Patients complete tools about their values and wishes, and a health care provider uses a structured discussion tool to talk about the patient's health condition and future wishes. The patient's family member/substitute decision-maker is encouraged to attend and be part of the discussions.

The ACCELERATION (ACtivity, smoking Cessation, healthy Eating and aLcohol Education, inteRvention, and motivATION) Program is a collaborative project in cancer and chronic disease prevention that has been designed by and will be delivered amongst cardiopulmonary rehabilitation and prevention programs and cancer centres in Ontario (ON), British Columbia (BC), Quebec (QC) and Nova Scotia (NS). This project is being funded by the Canadian Partnership Against Cancer, Health Canada and Heart and Stroke Foundation of Canada and in the amount of $2.4M over a 3 year period (October 2013 - September 2016). The ACCELERATION Program is a 12 week structured model of behavioural interventions and education around self-management and prevention that aims to access readily available and referable people for primary prevention intervention.This program aligns with current provincial, national and international chronic disease prevention and management strategies. The goal of the ACCELERATION program is to measurably and effectively change the risk factors and health behaviours known to impact cancer and other chronic diseases. Specifically we will aim to increase physical activity, reduce smoking, encourage healthy eating, and moderate alcohol consumption in about 3,000 participants across Canada over 3 years.

Patients with chronic lung diseases such as Chronic Obstructive Pulmonary Disease (COPD), who perform regular physical activity, have improved health and wellbeing compared with those who do little exercise. The purpose of the study is to evaluate whether the use of a simple pedometer (step counter) to set targets for daily physical activity can encourage COPD patients referred for an 8-week pulmonary exercise based programme (PR) to be more active. The investigators also want to know whether the use of pedometers during PR can improve adherence, self-management and outcome in COPD.
