Diabetic RetinopathyDiabetic Macular EdemaBackground: - Diabetic macular edema (DME) is a common condition in people with diabetes. DME occurs when blood vessels in the eye leak fluid, resulting in swelling inside the back of the eye and progressive vision loss. Research has shown that good blood sugar control can reduce the risk and severity of DME. However, not all diabetic patients with poor blood sugar control develop DME, and some patients develop DME despite excellent blood sugar control. This suggests that other factors, such as genes or inherited traits, may predispose or protect a diabetic patient from developing DME. Objectives: - To investigate genetic factors that may influence the development of diabetic macular edema. Eligibility: - Individuals at least 18 years of age who have type 2 diabetes, with or without diabetic macular edema. Design: The study will require one visit to the National Institutes of Health eye clinic. Participants will be screened with a medical history and basic eye examination. Individuals who have certain eye diseases other than DME may not be allowed to enroll in the study. Participants will provide a blood sample, and will receive fluorescein angiography (an injection of fluorescein dye, after which a camera will take pictures of the dye as it flows through the blood vessels in the eye). No treatment will be provided as part of this protocol.
Micropulse 810 Nanomolar (nm), Diode Laser for Diffuse Diabetic Macular Edema
Diabetic Macular EdemaObjective: To compare micropulse 810nm diode laser photocoagulation versus argon laser photocoagulation for treatment of diabetic macular edema. Micropulse laser technique will be determined by an initial clinical trial comparing single versus double density laser photocoagulation techniques for treatment of diabetic macular edema.The single density is based on the Early Treatment Diabetic Retinopathy Study (ETDRS) grid photocoagulation technique and the double density increases the number of spots. Methods: Patients with diabetic macular edema will be assigned to receive either micropulse 810nm diode laser photocoagulation or argon laser photocoagulation therapy. First, in a smaller clinical trial, patients will be assigned to single or double density micropulse 810nm diode laser to determine best strategy for this therapy. Visual acuity, fundus photographs and fluorescein angiography, and optical coherence tomography measurements , autofluorescence and mfERG were obtained at baseline and at 1, 3 and 6 months and 12 months.

Bone Marrow EdemaSpA is a chronic, debilitating inflammatory rheumatic disease that affects axial and peripheral joints, internal organs, and other tissues. Sacroiliitis is a hallmark of axial SpA. Sacroliliitis has a great effect on spine pain, function and cephalic progression of the disease . The prevalence of SpA ranged from 9 to 30 per 10,000 persons in Caucasian populations, Europe or the United States . The management of axial SpA is really challenging. The traditional disease-modifying anti-rheumatic drugs (DMARDs) were ineffective in controlling the axial disease. Biological agents such as tumor necrosis factor alpha (TNFα) inhibitor and anti-interleukin 17 have shown promising results in achieving remission or low disease activity for axial SpA . Magnetic resonance imaging (MRI) is established as the imaging method of choice for the early diagnosis and follow-up of SpA patients. Although a positive imaging detection of the axial skeleton is no longer obligatory based on the new Assessment of Spondyloarthritis International Society (ASAS) classification criteria 2010, MRI imaging still plays a crucial role, especially in the early diagnosis arm. MRI has a high sensitivity in detection of acute inflammatory processes as well as the high-resolution visualization of anatomical alterations. The lack of radiation exposure makes MRI ideal for monitoring response to treatment .Bone marrow edema (BME) not only showed high sensitivity for detection of early sacroilitis, but also its reduction was a worthy indicator for disease remission. Intra Articular (IA) injections of SIJ with corticosteroids and anesthestics are often performed for pain relief. Although this technique is relatively old, it was not used on a lrage scale in axial SpA patients. Further, its effect on disease outcome measures were not well elucidated. To the best of our knowledge there is no single study have evaluated effect of steroid and local aneshestic injection on improvement of BME . Image guidance of the SIJ injection is fundamanetal, due to the complex anatomy of the joint causing a low accuracy when performed using blind technique .

Effects of Infrasound Exposure on Measures of Endolymphatic Hydrops
Vestibular AbnormalityEndolymphatic HydropsPersons exposed to infrasound - frequencies below 20 Hz - describe a variety of troubling audiovestibular symptoms, but the underlying mechanisms are not understood. Recent animal studies, however, provide evidence that short-term exposure to low frequency sound induces transient endolymphatic hydrops. The existence of this effect has not been studied in humans. The long-term objective of this research is to identify a possible mechanism to describe the effects of infrasound on the human inner ear. The central hypothesis of the proposed study is that short-term infrasound exposure induces transient endolymphatic hydrops in humans. This will be tested by performing electrophysiologic tests indicative of endolymphatic hydrops among normal hearing individuals before and immediately after a period of infrasound exposure. Recordings of infrasound generated by wind turbines in the field have been established and calibrated by this team of engineers, otologist, and hearing and balance scientists. An infrasound generator reproduces the acoustic signature based on these field recordings. Aim 1: Determine the effect of infrasound on the summating potential to action potential (SP/AP) ratio on electrocochleography (ECoG). Hypothesis 1: Infrasound exposure will cause a reversible elevation of the SP/AP ratio. Aim 2: Determine the effect of infrasound on the threshold response curves of ocular and cervical vestibular evoked myogenic potentials. (oVEMP and cVEMP). Hypothesis 2: Infrasound exposure will cause elevation of the oVEMP and cVEMP thresholds at the frequency of best response. Successful completion of the aims will provide evidence for a possible mechanism of the effect of infrasound on the inner ear. This understanding will benefit individuals exposed to environmental infrasound and those in regulatory, research, and advocacy roles when crafting interventions and future policy.
