Detection of Latent Tuberculosis in Hemodialysis Patients

Long Term Treatment of End Stage Renal Disease Patients With Lanthanum Carbonate (Fosrenol)

Hyperphosphatemia

Patients who have been treated with Fosrenol for a minimum of 12 consecutive weeks and are receiving dialysis will be followed for 5 years to compare mortality, bone fractures and incidence of selected morbidities to patients with no lanthanum exposure and to patients being treated for hyperphosphatemia with any marketed product.

Many patients with chronic renal disease show a loss of the nocturnal decline of blood pressure (non-dipper). However, the mechanism is not yet fully understood. We evaluate 24-hour blood pressure in patients with chronic renal disease using an ambulatory blood pressure monitoring device (A & D TM2425). We also analyze the power spectrum of heart rate variability as an index of autonomic cardiovascular modulation using the same device.

The Lighthouse Tenofovir Cohort Study

Chronic Renal Disease

Lighthouse, in cooperation with the University Heidelberg Public Health Institute and the University Köln (Cologne) would like to set up a cohort to study baseline characteristics and long-term clinical outcomes of patients using Tenofovir based Antiretroviral Therapy at the Lighthouse. As of March 2014, patients above 18 years and giving informed consent coming to the Lighthouse to newly initiate ART will be approached to enroll in the cohort. The results will be disseminated both nationally at the Ministry of Health Technical Working Group (TWG), at the annual Research Dissemination and Best Practices conferences of the College of Medicine and National AIDS Commission as well as internationally. The results will also be written up for publication in appropriate peer-reviewed journals.

Assessment of Nutrition in ESRD

Normal growth can be divided into four important phases: prenatal, infantile, childhood and pubertal. Nutrition is important at all phases of growth, but particularly so during the infantile phase because the rate of growth is higher than at any other time of life and is less dependent on growth hormone than during other phases. During the childhood phase, growth becomes more dependent on the GH/insulin-like growth factor-1 axis; growth rate decelerates continuously until the pubertal phase. The pubertal phase results from the coordination of GH and sex steroid production. Together they have an anabolic effect on muscle mass, bone mineralization and body proportions. It is another phase of rapid growth so that nutrition can again modify the genetic growth potential.

The evaluation and assessment of the influence of the different modalities of renal replacement therapy (RRT), including peritoneal dialysis (PD), conventional hemodialysis with high flux dialyzers, hemodiafiltration (HDF) and expanded hemodialysis (HDx) with Medium Cut Off (MCO) dialyzers on the levels of the cell subsets of the innate and acquired immune system as well as the investigation of the possible role of these cells as prognostic indices of morbidity and mortality in patients with end-stage Chronic Kidney Disease (CKD) undergoing different modalities of RRT.

Patient-Centered Outcomes for One-stage and Two-stage Brachial Basilic Arteriovenous Fistulas

End Stage Renal Failure

The absence of reliable data to establish the optimal surgical approach for creating a new Brachial Basilic Arteriovenous Fistula (BBAVF) has resulted in high degree of treatment equipoise and the decision to use one-stage or two-stage approach is largely based on surgeon's preference. The pilot trial will provide a framework for an evidence-based surgical approach to create a BBAVF for patients receiving renal replacement therapy. The pragmatic trial design will allow the inclusion of different surgical techniques currently used by the access surgeons to create a new BBAVF. Subjects in whom randomly assigned approach (one-stage or two-stage) is not completed will be considered appropriately treated as intended to account for the risk of primary fistula failure. The investigator will use the patient-centric clinical endpoints and patient reported outcomes (PROs) to incorporate the perspective of patients undergoing complex The goal is to prospectively evaluate the impact of the one-stage and two-stage BBAVF approaches on qualitative quality of life (QOL) in order to address the knowledge gap within the existing literature on complex arterovenous fistula (AVF) procedures. The investigator will use the NIH-sponsored Patient-Reported Outcome Measurement Information System (PROMIS) Computerized Adaptive Testing (CAT) tool to evaluate self-reported measures for functions, symptoms, behaviors, and feelings following the BBAVF procedure
