A Study of Alpelisib and Fulvestrant to Treat Endometrial Cancer

Endometroid Endometrial Cancer

This is a 2 stage multi-center study designed to evaluate the efficacy of the combination of alpelisib and fulvestrant in patients with PIK3CA-mutated ER-positive endometrioid endometrial cancers by estimating the objective response rate (ORR). Treatment will continue until either unacceptable toxicity, progression of disease, or investigator/patient request for withdrawal.

The primary purpose of this study is to compare three interventions, two experimental and one standard of care (usual care), to see if the experimental interventions will increase the likelihood of a participant obtaining guideline-concordant genetic testing. Eligible participants will be randomized (assigned) to one of the following interventions: 1) Virtual genetics navigator, a mobile-optimized website, designed by the investigators, that delivers tailored messages and content; 2) two motivational interviewing (MI) telephone calls delivered by trained genetics health coaches; or 3) usual care.

The goal of this observational study is to learn about the added diagnostic and prognostic value of advanced medical imaging procedures in cervical cancer, endometrial cancer and ovarian cancer. The main questions it aims to answer are: Does advanced medical imaging predict survival? Can advanced medical imaging improve radiotherapy target planning? Are advanced medical imaging results associated with risk markers found in tumor tissue? Participants will Undergo four additional imaging procedures, as compared to clinical routine examinations, two at baseline and two after three months. Be subject to clinical follow-up for five years.

Multicenter, randomized, open-label, phase II clinical study comparing Dostarlimab +/- Bevacizumab with standard chemotherapy in patients with gynecological clear cell carcinoma. 198 subjects will be enrolled in this study and will be assigned to three groups in a 1:1:1 ratio. Group A: Dostarlimab monotherapy First 3 cycles: Dostalimab 500mg every 3 weeks, IV 4 cycles ~ up to 24 months: Dostalimab 1000mg every 6 weeks, IV Group B: Dostarlimab + Bevacizumab combination therapy First 3 cycles: Dostalimab 500mg every 3 weeks, IV 4 cycles ~ up to 24 months: Dostalimab 1000mg every 6 weeks, IV Bevacizumab administered IV at 15 mg/kg every 3 weeks until disease progression or unacceptable toxicity Group C: General chemotherapy (one of doxorubicin, paclitaxel, and gemcitabine)

In this context, the aim of this study is to investigate the role of MRF in endometrial cancer. Several applications are possible. Firstly, T1-, T2- and DWI-mappings can be associated to molecular risk group classification, in order to stratify patients' risk without recurring to surgery. MRF parameters can be also correlated to prognosis, both in terms of disease-free survival (DFS) and overall survival (OS).
