Evaluate the efficacy and safety of testosterone supplementation on functional capacity, biomarkers, quality of life and clinical outcomes for patients with heart failure.

Sex has a major impact on myocardial metabolism and blood flow. In subjects without heart failure men's hearts tend to use proportionally more glucose and women's hearts use more fat and have higher blood flow. Obesity is a major risk factor for Heart Failure. In subjects without heart failure, increasing body mass index is predictive of increased blood flow and fatty acid metabolism in women, but not men. To measure blood flow and metabolism we will be using radioactive materials and a PET (positron emission Tomography)scan to study the blood flow and substrate metabolism of the heart. Hypotheses: 1) Women with heart failure with reduced ejection fraction (HFreF) will have higher levels of heart blood flow and fatty acid metabolism and lower glucose metabolism rates than men with HFrEF. A secondary Aim is test the hypothesis that body mass index (BMI), a measure of obesity, correlates with myocardial blood flow and myocardial metabolism measures in patients with HFrEF.

Cardiac MR (CMR) will be used to assess myocardial activation and hemodynamics in patients with CRT. Goals are to demonstrate that differences in Left ventricle (LV)- right ventricle (RV) pacing delays cause differences in myocardial activation differences in LV-RV pacing delays cause differences in LV/RV hemodynamics and to use these data to optimize CRT.

Brain natriuretic peptide (BNP) is a useful therapy when treating patients with heart failure. As many of these patients also have airways disease it is important to determine if BNP also has a positive effect on their respiratory condition. The role of BNP in airways disease has never been studied although there is evidence to suggest that it will have a positive effect. The current study is therefore a proof on concept study which will demonstrate whether BNP (nesiritide) will improve both heart failure and airflow obstruction in patient who have both. The investigators are looking to enroll 40 patients with heart failure and airways disease who present to hospital emergency departments. Patients who consent and meet the entry criteria will be randomised to receive either nesiritide or placebo in addition to standard therapy. They will receive a bolus of study medication followed by a 4 hour infusion. Before, at hourly intervals and immediately following the infusion the following data will be collected: dyspnoea score respiratory rate FEV1 (if able to be performed) peak respiratory flow rates (PEFR, if able to be performed) requirement for concomitant bronchodilator therapy urinary GMP At all times during the study period and at the conclusion of the study patients will be provided with the best available therapy for their condition at the physicians' discretion.

The primary objective is to measure the association between extracellular RNA (ex-RNA) levels in plasma in patients receiving aggressive outpatient therapy for CHF with (1) cardiac remodeling and (2) cardiovascular events. The investigators will follow patients during standard medical therapy for CHF to assess changes in ex-RNA levels in the plasma, and how these are associated with cardiac remodeling (by cardiac imaging) and outcomes.

The purpose of this study is to evaluate the efficacy of ranolazine in the prevention of recurrent atrial fibrillation in post-cardioversion patients with heart failure and preserved ejection fraction.

The primary efficacy objective of this study is to evaluate the effect of ferric carboxymaltose on mitochondrial gene activation pattern after 12 weeks of treatment.

Clinical trial phase IIb, double-blind, randomized, controlled with placebo. There is sufficient preliminary evidence to consider intracoronary injection of bone marrow progenitor cells as a viable, safe and beneficial treatment in patients with dilated cardiomyopathy, although the biological mechanism of action of bone marrow cells in the myocardium is not known. In this project we propose to investigate comparatively and from a biological and clinical point of view the applicability of regenerative therapy with autologous bone marrow cells in patients with dilated cardiomyopathy.

The purpose of this study is to assess the efficacy and safety of implanting an Edwards Sapien XT Valve into the vena cava inferior (VCI; between right atrium and the hepatic vein) on clinical variables, exercise tolerance and well being in patients with severe tricuspid regurgitation and signs of right heart failure.
