Idiopathic Dilated Cardiomyopathy

Clinical trial phase IIb, double-blind, randomized, controlled with placebo. There is sufficient preliminary evidence to consider intracoronary injection of bone marrow progenitor cells as a viable, safe and beneficial treatment in patients with dilated cardiomyopathy, although the biological mechanism of action of bone marrow cells in the myocardium is not known. In this project we propose to investigate comparatively and from a biological and clinical point of view the applicability of regenerative therapy with autologous bone marrow cells in patients with dilated cardiomyopathy.

Heart Failure (HF) a common clinical condition characterized by either by a heart that does not pump sufficiently or becomes stiff. A variety of mechanisms contribute to progressive cardiac remodeling and dysfunction. A new therapeutic approaches by preventing activation of the brain neuromodulatory pathway, may lead to improve HF. QCG001 is a prodrug of EC33, a aminopeptidase A (APA) inhibitor. QCG001 has been shown to be an antihypertensive agent in animal models. This study investigates the safety and efficacy of QGC001 in HF patients.

Trial of an Internet-based Platform for Monitoring Heart Failure Patients

Several studies have indicated that heart failure (HF) clinics facilitating patient self-management and using multi-disciplinary care reduce in hospital bed-days, all-cause and HF-related hospitalizations, all-cause mortality, and improve patient quality of life. Additionally, national and provincial organizations have identified patient-focused homecare initiatives in telehealth as demonstrating great promise for health service. Our pilot study demonstrated the feasibility of the internet based vHFC and supports the investigation of this intervention in patients with HF. The purpose of our single-blinded randomized trail is to investigate the efficacy of an Internet-based heart failure (HF) clinic (vHFC) in 186 patients living with HF. Hypotheses A. Participation in a vHFC emphasizing patient self-management and monitoring will result in improved functional capacity compared to usual care in patients with HF. B. Participation in a vHFC emphasizing patient self-management and monitoring will result in improved health indices such as, self-management skills, quality of life, levels of B-type natriuretic peptide and healthcare utilization compared to usual care in patients with HF. Our objectives of the vHFC study is as follows: To establish a cohort of 186 patients with HF. To determine the benefits of participating in the vHFC over usual care at 12 months with respect to exercise capacity. To compare the changes in other risk factors and lifestyle behaviours between the vHFC and usual care patients.

Sex and Obesity: Effects on Heart Failure Study

ObesityHeart Failure

Sex has a major impact on myocardial metabolism and blood flow. In subjects without heart failure men's hearts tend to use proportionally more glucose and women's hearts use more fat and have higher blood flow. Obesity is a major risk factor for Heart Failure. In subjects without heart failure, increasing body mass index is predictive of increased blood flow and fatty acid metabolism in women, but not men. To measure blood flow and metabolism we will be using radioactive materials and a PET (positron emission Tomography)scan to study the blood flow and substrate metabolism of the heart. Hypotheses: 1) Women with heart failure with reduced ejection fraction (HFreF) will have higher levels of heart blood flow and fatty acid metabolism and lower glucose metabolism rates than men with HFrEF. A secondary Aim is test the hypothesis that body mass index (BMI), a measure of obesity, correlates with myocardial blood flow and myocardial metabolism measures in patients with HFrEF.
