Visual Outcomes of Vivity in Patients With Well Controlled Glaucoma
Patients With Primary Open Angle Glaucoma (POAG) or Ocular Hypertension (OHT)Glaucoma is a progressive optic neuropathy that results in a loss of contrast sensitivity and visual field if not detected and treated. When glaucoma patients undergo cataract surgery, they are often not ideal candidates for many existing presbyopia correcting IOLs as they reduce contrast sensitivity because of the splitting of light and are more likely to have visual disturbances secondary to diffractive steps in the IOL design. This is largely because of a higher rate of ocular surface disease and meibomian gland dysfunction in this patient population due to the chronic utilization of topical drops. As a result of this, patients with glaucoma currently receive aspheric monofocal IOLs during cataract surgery, which are lenses with minimal loss of contrast sensitivity but only one point of focus (typically targeted for distance). This necessitates the use of glasses for near and intermediate vision and has an impact on quality of life for these patients. The Vivity Extended Depth of Focus (EDOF) IOL is a new technology that maintains uncompromised distance vision and provides improved intermediate vision correction, reducing the need for glasses. This lens uses a new optical system with no diffractive steps in the IOL; trials in non-glaucomatous patients have shown the rate of visual aberrations to be comparable to an aspheric monofocal IOL which are currently the standard of care in patients with glaucoma. There is no published data on outcomes of this IOL in patients with primary open angle glaucoma (POAG) or ocular hypertension (OHT). Studies are required to evaluate the clinical success of this new lens technology in glaucomatous patients. This will be the first study in Canada to report clinical outcomes of the Vivity IOL in this patient cohort.

Despite new non-invasive measurement methods most of the children are uncooperative during the ophthalmological examination. Therefore examination under anaesthesia is needed. A lot of perioperative factors influence the measurement of intraocular pressure. Established and safe anaesthetic regimes have been modified regarding these factors. Aim of the study is to evaluate, if a standardized anaesthetic protocol generates reliable and reproducible measured values.

Intra Ocular HypertensionThe purpose of this study is to examine whether the intraocular pressure of operated people under general anesthesia is affected by the artificial respiration method. If it is proven that there is such influence, the findings may lead to select the best method for patients with increased intraocular pressure. In this study, the investigators will monitor the changes observed in intraocular pressure caused during an operation in two different artificial respiration methods: Intermittent positive pressure ventilation (IPPV) High frequency jet ventilation (HFJV) Study recruited 50 patients aged 18 - 40, candidates to elective orthopedic surgery in the limbs. These patients do not suffer from general diseases that restrict their daily functioning. The selection of the type of surgery was done to prevent the involvement of any other factor, except the respiration method, determination of intraocular pressure. Exclusion Criteria: Patients who are unable to provide an Informed Consent Patients suffering from any eye disease Patients with corneal thickness less than 450 microns or more than 600 microns Smokers Patients with pulmonary disease restrictive or obstructive Patients using bronchodilator All patients operated under general anesthesia using total intravenous anesthesia )TIVA) method. All patients will be resuscitating half of the time of surgery using IPPV, and in the second half of surgery using HFJV. During the surgery the IOP will be measured by an ophthalmologist who is unaware of the respiration method. Correction will be made to the intraocular pressure values, depending on the thickness of the cornea. The results of the measurements will be collected and the clinical significance will be reviewed by acceptable statistical methods.
Evaluation of Rate of Progression With Perimetry in Newly Diagnosed Open Angle Glaucoma
GlaucomaOpen-Angle1 moreThe purpose of this study is to evaluate rate of progression in newly diagnosed open angle glaucoma with different types of perimetry. Would it be possible to find the rapidly progressing individuals sooner by doing perimetry more often? Is there a difference between different types of perimetry in early glaucoma? What would be the optimal timing of perimetry in newly diagnosed glaucoma? In addition the quality of life in glaucoma patients will be evaluated with two types of questionnaires in three different time points. The enrolled patients will receive appropriate treatment according to glaucoma management guidelines.
