Quality control and improvement project: Assessment and analysis of processes and outcome in trauma emergency room and major trauma patients at the Aarau trauma center (cantonal hospital) with regard to initial emergency management, hospital processes and short- and long-term (1-and 2-year) outcome of patients (subjective and objective). Comparison of processes and outcome pre- and post- project initiative. Benchmarking with the literature and by participation in the German and Swiss Trauma registry each.

The major aim of this project is to investigate the prevalence, recovery patterns and risk factors for health related quality of life functional, psychological, societal and economic outcome after trauma. Another aim is to validate the WHO Quality Of Life-bref (WHOQOL-Bref) questionnaire for the trauma population.

The Role of Fibrocytes in Acute Lung Injury

Acute Lung Injury(ALI)

The important character of acute lung injury (ALI) is alveolar capillary membrane damage caused by different diseases, such as sepsis, trauma and shock. One of the important pathological stages is the varying degrees of interstitial fibrosis and semi-permeable alveolar membrane fibrosis. It has been proved that CXCL12/SDF-1 (stromal cell-derived factor-1) induces fibrocyte migration, and promotes fibrosis progression. Study indicated that inhibition of TLR4 receptor signaling pathway improves fibrosis progression induced by ALI, however, the role of fibrocyte in ALI is still unclear. The fibrocytes was significantly increased in asthmatic patients with pulmonary fibrosis, which companies with increased CTGF expression. Therefore, this project assumes that fibrocyte will differentiation to fibroblast/myofibroblast in patient with acute lung injury, which in turn leads to progression of fibrosis. The central hypothesis of this project is that peripheral progenitor cell fibrocytes play an important role in alveolitis caused by acute lung injury. The overall objective of this project is to study the role of fibrocytes in acute lung injury.

Stair negotiation is an important activity of daily living. Patients with cruciate ligament injury may have difficulty in stair negotiation because of the proprioception deficit and neuromuscular deficit caused by ligament injury. However, the influence of knee cruciate ligament injury on the control of movement is still unclear. Since inter-joint coordination can provide the information of relationship between angle position and angle velocity, it may be useful to analysis the change of movement pattern.This current study aimed to investigate the change of inter-joint coordination and muscle activation in patients with cruciate ligament injury during stair negotiation.

Injury

Judo is one of the possible items in our country to win the first prize in Olympics. However, scientific research in this field is not enough to support the coaches to train the athletes. Series of research should be planned and executed to help the coaches and to promote the performance of our athletes. This is the first step of the serial research. The purposes of this study are: in the first year, to find the correlations between the physiological/psychological competitive abilities of the second/third year athletes and their performance/injury status. And, the physiological/ psychological competitive abilities of the first year athletes will be collected in the beginning of this year. And then, their performance/injury status will be followed through the whole year and to the end of the second year monthly. The purposes of this study in the second year are to find the causation of the physiological/ psychological competitive abilities in the first year with the performance/injury status in the second year. The subjects of this study are from 4 senior high schools in the north part of Taiwan which performed well in national judo competition in senior high schools in the past years. The study designs are both cross-sectional and prospective. In the part of the second/third year athletes, it is a cross-sectional design, and in the part of the first year athletes, it is a prospective design. The physiological status will be assessed with their general fitness and balance. The fitness test includes body composition, strength, power, endurance, agility, coordination, balance, flexibility, and cardiopulmonary capacity. The psychological competitive abilities will be assessed with a questionnaire. Injury and the possible causes will also be collected with a questionnaire monthly. Finally, the data will be analyzed with descriptive analysis, chi-square test, t-test and regression analysis to show their characteristics, correlations and causations.
