A Pilot Study of Bevacizumab for Neoplastic Meningitis

Neoplastic Meningitis

The goal of this clinical research study is to learn if and how Avastin (bevacizumab) may affect cancer that has spread to the meninges of the brain or the spinal cord. The safety of this drug will also be studied. Objectives: 1. Primary: 1. Determine preliminary response data of intravenous bevacizumab in patients with NM a. As measured by clearance of malignant cells from the Cerebrospinal fluid (CSF) at 2, 4, 6, 12, 18, and 24 weeks, then every 8 weeks up to 54 weeks, and b. Time to neurological progression (TTNP) 2. Secondary: Evaluate the safety of intravenous bevacizumab in patients with NM Further describe the efficacy of this intervention as measured by improvement of MR imaging evidence of disease overall survival maintenance of quality of life Determine effects of systemically administered bevacizumab on CSF, serum, and urine Vascular endothelial growth factor (VEGF)levels levels Correlate changes in CSF VEGF with response measurements. Correlate primary tumor tissue VEGF expression with CSF VEGF levels Correlate urine VEGF levels with serum and CSF VEGF levels Evaluate serum and CSF VEGF index

Leptomeningeal Metastasis

It has been proved that concurrent radiotherapy (RT) and intrathecal methotrexate (MTX) for leptomeningeal metastases (LM) from solid tumors with adverse prognostic factors showed great effectiveness and safety. Cytarabine(Ara-C) is another agent which is commonly used for intrathecal chemotherapy. The purpose of the study is to observe the effectiveness and safety of concurrent RT and intrathecal chemotherapy for LM from solid tumors. In addition, the effectiveness of these two types of agents (MTX and Ara-C) in the concurrent chemo-radiotherapy will be compared in this study. This is a randomized controlled, parallel group, and phase II clinical trial. The object of this study is newly diagnosis patients with leptomeningeal metastases from solid tumors, who will accept the treatment of involved-field RT combined with concurrent intrathecal-MTX or intrathecal-Ara-C, respectively. Major endpoint is clinical response rate. Secondary endpoints are time to progression,severe adverse events and overall survival.

To find out what effects (good and bad) topotecan has when given directly into the cerebrospinal fluid in children with neoplastic meningitis (cancer that has spread to the lining of the brain and spinal cord). Cerebrospinal fluid is the fluid that circulates around the brain and spinal cord. To determine if intrathecal topotecan is beneficial to patients. To better understand how topotecan is handled by the body after intrathecal administration. To evaluate the cerebrospinal fluid for signs (markers) of tumor spread.
