Musculoskeletal DiseasesThe purpose of this study is to create a research repository, comprised of data collected in the course of physical rehabilitation programs for musculoskeletal disorders (MSDs) using the medical device SWORD Phoenix®. This will allow the company to ascertain the acceptance, engagement and effectiveness of programs using this asynchronous tele-rehabilitation platform in the treatment of multiple MSDs.
Improve the Quality of Care in Patients With Orthopaedic Disorders
Orthopedic DisorderThe main objective of the investigators is to improve the quality of care in patients with orthopedic disorders followed in St Luc hospital (Brussels Belgium). To do this, the investigators want to assess the impact of Orthopedic treatments at the structural level (bone structure, muscle, etc.), at the functional level (mobility, strength, stiffness...), on the restriction of activities of patients (walking, make its care daily..) and on the limitation of participation in the life of every day (sport, work, social life, cultural...). This functional evaluation of patients with orthopedic disorders by the ICF model is an original approach rarely used in muscular-skeletal impairments that can very improve the management of these patients and their quality of life. In addition, the investigators associate the harvesting of all medical and computer data collected by high-precision tools in the surgical treatments, to better define the surgical precision and improve the quality of surgical care.
Multi-Vendor Multi-Site Novel Accelerated MRI Relaxometry
Bone DiseasesInfectious5 moreCurrently the diagnosis of OA is based on radiographs and clinical findings, which is limited to detecting late-stage disease. There is a pressing, unmet clinical need for robust assessment of early changes in cartilage health. Towards this goal, extensive efforts are ongoing to develop quantitative MRI for cartilage matrix analysis. MR T1ρ and T2 relaxation times have shown to be promising imaging biomarkers for early cartilage degeneration and prediction of disease progression. However, many challenges remain to clinically applying these techniques, including lack of standardized acquisition and quantification methods, and long acquisition times. The study aims to develop novel, fast and reproducible MR T1ρ and T2 relaxation time imaging methods on MR systems from multiple vendors and establish a platform for standardization and cross validation of these measures as a tool for clinical trials using such techniques. Following method validation, patients at risk for osteoarthritis will be tested.
PaO2 and Lung Function After Orthopedic Surgery
Postoperative ComplicationsOrthopedic DisorderHypoxia and reduced oxygen partial pressure is commonly occurring after abdominal surgery. This study aims to investigate whether similar changes also occur after orthopedic surgery in the form of upper limb surgery. Inclusion: 60 patients undergoing orthopedic surgery in the form o knee-, hip-, shoulder- or elbow surgery. Exclusion: Dementia or cognitive impairment that makes it impossible to participate in studies. Arterial blood gas and lung function are undertaken before surgery, the day after surgery and at follow-up.

The population growth and aging society subsequently increase the number of disabled people, the healthcare costs and the needs for healthcare professionals. The evidence exists to support the beneficial effect of exercises on function and quality of life. Traditionally, a rehabilitation program is designed by therapists for each patient depending on their conditions. In recent years, AI is increasingly being employed in the field of physical and rehabilitation medicine, however, there is no study of applying AI in predicting rehabilitation programs for shoulder musculoskeletal disorders. The main purpose of this study is to explore the possibilities of using supervised machine learning approach to predict rehabilitation programs for shoulder musculoskeletal disorders. Twenty-three features are identified based on shoulder range of motion, pain, whether or not perform surgical procedure. Each exercise is considered as a label with a total of twenty-five exercises. Dataset is collected by clinical therapists to develop and train the model. Each patient has to receive at least two months of rehabilitation and two times of evaluation. Logistic regression, support vector machine and random forest are used to build the computational model. Accuracy, precision, recall, F-1 score and AUC are used to evaluate the performance of the computational model in machine learning. After training, we compare the consistency of rehabilitation programs predicted by using machine learning model and the clinical decision making of therapists.

As a result, sitting has become the most common learning posture and office posture, and cervical and lumbar spondylosis are becoming increasingly prevalent in younger people. One-third of life is devoted to sleep, and bad sleeping posture can place incorrect strain on the cervical and lumbar spine, resulting in irreversible and detrimental repercussions if not treated seriously. Sleep disorders have become an important public health concern because they can have detrimental effects on both mental and physical health. A pillow can give adequate support for the head and neck and assist sleepers in maintaining proper neck and thoracic curvature. Studies have demonstrated that a comfortable sleeping pillow helps relax the neck muscles, hence facilitating sleep, and efficiently alleviate pain in the neck, shoulders, back, and head. There is consensus among researchers that supporting the natural lordotic curve of the cervical spine is necessary to achieve longer periods of deep sleep. In addition, a pillow can improve sleep quality by cooling the head, lowering body temperature, reducing sweating or slowing the heart rate during sleep. However, the level of evidence to support these claims is so far limited. It has been claimed by pillow manufacturers that many design-related pillow parameters are effective in improving sleep quality and reducing neck pain. However, most of these recommendations are based on personal experience. This study recommends the most suitable pillow for people in forward head postures based on the body pressure distribution of the head, neck and upper body. The proposed pillows were evaluated in two parameters: comfort and support.

Virtual Reality - A New Vision on Pain
Chronic PainPain6 moreThe purpose of the project is to investigate if the use of virtual reality (VR) technology with pre-programmed proprietary software can provide better treatment for patients with long-term pain conditions. This is investigated in this pilot study conducted on patients referred to outpatient treatment at the Division of Physical Medicine and Rehabilitation. The pilot study will lay the foundation for a follow-up study (RCT). The effect of VR are evaluated by patient reporting forms; before start-up and 3 months after startup. Patients included in the study receive standard treatment supplemented with the use of VR technology. The therapists involved will be Interviewed to examine their experiences with the use of the VR-technology in the treatment of patients. VR technology can potentially contribute to better treatment (e.g. measured in outcomes such as activity, sleep problems, pain intensity, quality of life). Moreover we expect that a VR-assisted treatment to be more cost-effective and increased availability regardless of geography and demography.
Accelerometry and Rehabilitation After Knee Replacement Study (ARK)
Orthopedic DisorderTotal knee replacement (TKR) is a successful and cost-effective treatment for end-stage arthritis. Its usage is increasing due to changing population demographics and quality of life (QoL) expectations. There were almost 100,000 total knee replacements (TKR) performed in England between 2018/19. After TKR, patient engagement in their rehabilitation exercises is very important but physiotherapy services are very stretched at present especially in the covid-19 era. Patient compliance with their exercises is poor at about 25%. The use of wearable sensors (WS) following TKR is gaining lots of interest especially in the post covid-era. A recent review found five small studies which supported the feasibility of their use. WS may help with patients to engage better with their rehabilitation exercises after surgery, delivering remote physiotherapy and potentially help identify patients who may be struggling more and therefore need further targeted help with physiotherapy. Sensors have the potential to be a cost-saving intervention for the NHS by improving efficiencies in monitoring patients by reducing number of outpatient appointments, reducing rehabilitation time, improving patient's adherence to rehabilitation schedules and increasing confidence in exercise regimes leading to improved health-related quality of life. We will conduct the first large scale study where 250 patients having TKR will be randomly chosen to either receive a WS (125 patients) against 125 patients who will have standard care (SC). Patient reported outcomes (PROMs), pain scores, objective measures of knee function and data from the sensors on how well patients engaged with their exercises will be collected. The study will also assess if WS is cost effective, at 6 months after surgery. This study has the potential to revolutionise how pre- and post-knee replacement rehabilitation is delivered, providing an individualised, cost effective and successful solution to the current status.

Given the vital role fixation devices play in maintaining alignment, promoting healthy bone healing and preventing joint degeneration, it is essential to understand the expected lifetime outcomes of these implants, and evaluate their safety and efficacy. Prospective implant efficacy and safety registries are needed to support this endeavour, especially considering new regulatory requirements from the European Union Medical Devices Regulation (EU MDR) in relation to post-market clinical follow-up (PMCF).
