There exists substantial evidence suggesting that patients diagnosed with MSI-H/dMMR colorectal cancer can derive benefits from immunotherapy in the management of advanced colorectal cancer. In cases of locally advanced colorectal cancer exhibiting microsatellite instability (dMMR/MSI-H), patients exhibit low responsiveness to neoadjuvant chemotherapy, resulting in minimal rates of complete tumor remission and downstaging. Nevertheless, initial exploratory studies, characterized by modest sample sizes, reveal a favorable therapeutic effect of neoadjuvant immunotherapy in this particular patient population. Envality monoclonal antibody, the first PD-L1 antibody developed and manufactured in China, possesses noteworthy practical and societal value in the context of exploratory clinical research on neoadjuvant immunotherapy for locally advanced MSI-H/dMMR colorectal cancer patients. The objective of this study is to evaluate the safety and efficacy of Envality monoclonal antibody (PD-L1) as neoadjuvant therapy for locally advanced MSI-H/dMMR colorectal cancer through a prospective, multi-cohort phase II clinical trial. Additionally, this study aims to investigate the effectiveness and safety of Envality monoclonal antibody in combination with CAPEOX as a neoadjuvant treatment regimen for locally advanced pMMR colorectal cancer.

Tocotrienol and Bevacizumab in Metastatic Colorectal Cancer

This double blind, randomized phase II trial will investigate whether the addition of tocotrienol will improve the effect and lower the toxicity of standard chemotherapy and bevacizumab. Half of the patients will receive tocotrienol and the other half placebo. Treatment is planned for a period of maximum six months and will be discontinued earlier in case of progression or unacceptable toxicity.

The current clinical trials and data on the triplet regimen combined with bevacizumab for first-line treatment of metastatic colorectal cancer were from European and American populations. The triplet regimens recommended by the NCCN and ESMO guidelines using irinotecan and 5-FU at a higher dose intensity cause a high incidence of adverse events in Asian population, and there was no high-quality data on efficacy in Chinese population, both of which have limited the clinical applications of the regimens in China. This study intends to conduct an improved triplet regimen (mFOLFOXIRI) combined with bevacizumab versus mFOLFOX6 combined with bevacizumab as a first-line multicenter, randomized, controlled phase III clinical trial in patients with advanced colorectal cancer. Progression-free survival (PFS), observable response rate (ORR), overall survival (OS), disease control rate (DCR), surgical resection rate, and safety and health-related quality of life (HRQoL) were assessed in the two groups of subjects.

Study of Oral Metronidazole on Postoperative Chemotherapy in Colorectal Cancer

Fusobacterium nucleatum (Fusobacterium nucleatum, Fn) has been identified as an independent risk factor for recurrence of colorectal cancer. In this study, oral metronidazole would be used to reduce the abundance of Fn in patients with high Fn, so as to explore whether oral metronidazole can improve the efficacy of postoperative chemotherapy in patients with colorectal cancer.

mFOLFOXIRI Versus mFOLFOX6 as Adjuvant Chemotherapy for Locally Advanced Colorectal Cancer

The current standard treatment for locally advanced rectal cancer is still fluorouracil-based neoadjuvant radiotherapy and chemotherapy followed by TME surgery, followed by adjuvant chemotherapy. Fluorouracil single-agent simultaneous sensitization of radiotherapy and chemotherapy has a pCR of about 15-20% and a tumor downgrade (ypStage 0-I) rate of about 35%. However, about 30% of patients still have distant metastasis, which is the main obstacle affecting the survival prognosis of patients with locally advanced rectal cancer. About 50% -65% of patients was still stage II-III after neoadjuvant therapy. The long-term follow-up shows that for patients with ypT4 after surgery, the 3-year DFS is about 50%. For patients with ypN2, the 3-year DFS is less than 40%. Therefore, it is necessary to strengthen postoperative adjuvant chemotherapy to improve the survival prognosis for these patients. Although FOLFOX adjuvant chemotherapy improved survival benefit than 5FU as adjuvant treatment in ypStage II-III patients after neoadjuvant treatment in ADORE trial. However, with the progress of neoadjuvant therapy research, more and more studies have proposed to move part or all of postoperative adjuvant chemotherapy to preoperative neoadjuvant therapy due to low compliance of adjuvant chemotherapy. During neoadjuvant treatment, induction chemotherapy with FOLFOX / CAPEOX or consolidation therapy after CRT with FOLFOX / CAPEOX had been investigated a lot. The pCR rate was 19% -38%, and the tumor downstaging rate was about 50%. Another 50% of patients still had ypstage II-III postoperatively. The 3-year DFS for ypStage III was only 55% even with FOLFOX as adjuvant chemotherapy. And for ypT4N0 patients with ypstage IIB-IIC, there is also a higher risk of recurrence and metastasis. And it is urgent to explore new treatment strategies to improve this part of patients Survival prognosis. For locally advanced colon cancer, surgery combined with postoperative adjuvant chemotherapy is currently the standard treatment mode for stage II-III colon cancer. About 30% of patients with locally advanced disease will relapse within 3 years, of which distant metastases are more common and eventually become the main cause of death of patients. For locally advanced colon cancer with a preoperative staging of T4b, the NCCN guidelines recommend surgery after neoadjuvant chemotherapy with FOLFOX or CAPOX regimens. In the FOxTROT study of neoadjuvant treatment of locally advanced colon cancer, for patients with T3> 5mm or T4, after 4 courses of neoadjuvant chemotherapy with FOLFOX regimen, 20.5% of patients still have T4 after surgery, and 15.2% of patients had N2 disease. For this part of patients, new postoperative treatment options should also be explored to improve patient survival and prognosis. In view of the high efficiency of the three-agent FOLFOXIRI regimen in advanced colorectal cancer and the success in adjuvant chemotherapy after pancreatic cancer surgery, 5FU, oxaliplatin combined with irinotecan may have a synergistic effect. At present, a phase III randomized controlled study (IROCAS study) in Europe is underway. For high-risk phase III patients, the mFOLFOXIRI regimen is compared with mFOLFOX6 regimen adjuvant chemotherapy. Based on the above reasons, our center plans to further carry out "multi-center, randomized, controlled phase III clinical study of mFOLFOXIRI versus mFOLFOX6 adjuvant chemotherapy after neoadjuvant oxaliplatin in locally advanced colorectal cancer." Improve the survival prognosis of postoperative high-risk colorectal cancer patients after neoadjuvant therapy.

Non-commercial, prospective, randomized, multicenter, national, phase II, open-label comparative clinical trial. The patients will be randomized in a 1: 1 ratio in two arms: Control arm. Systemic chemotherapy with FOLFOX6m + monoclonal Ab Experimental arm. Systemic chemotherapy with FOLFOX6m + monoclonal Ab + Intra-arterial liver chemotherapy with LIFEPEARLS-IRINOTECAN (catheterization and infusion of 100 +/- 50 micron microspheres loaded with 100 mg of irinotecan in both liver lobes) cycles 2 and 4. The main objective is to evaluate the radiological objective response rate according to the RECIST version 1.1 criteria at 6 months. Secondary objectives include: Evaluate overall survival, progression-free survival (PFS), safety profile, hepatic PFS, R0 liver surgery rate.

The study will use previously established doses of panitumumab or cetuximab in the metastatic setting for the treatment of unresectable colorectal cancer (CRC). It is designed to investigate an alternative treatment strategy to maximize the benefit to inhibition of epidermal growth factor receptor (EGFR) for a highly selected patient population. It will enroll 110 participants with left-sided, unresectable metastatic CRC. Participants will be on study up to 5 years.

This phase Ib/II trial investigates the side effects and best dose of LY3214996 when given together with cetuximab alone or in combination with abemaciclib and to see how well they work in treating patients with colorectal cancer that cannot be removed by surgery (unresectable) and/or has spread to other places in the body (metastatic). Cetuximab is a monoclonal antibody that may interfere with the ability of tumor cells to grow and spread. LY3214996 and abemaciclib may stop the growth of tumor cells by blocking some of the enzymes needed for cell growth. Giving LY3214996 and cetuximab alone or in combination with abemaciclib may help treat patients with colorectal cancer.
