Medical Clowns for Pediatric Blood Draw

The research objective of this proposed clinical trial is to determine the efficacy of a medical clowning intervention for pediatric patients undergoing venipuncture. Efficacy is defined in terms of decreasing anxiety, pain, and crying duration, and increasing the pace and ease of the procedure. The study population includes pediatric patients between the ages of 3 - 11 years who must undergo venipuncture at the LAC + USC Outpatient Clinic. The subjects will be randomized into two groups. The control subjects will receive no intervention during blood draw, while the intervention subjects will receive the medical clown intervention during blood draw. The clowns will interact with one patient at a time, engaging in play with the patient and caretakers during all parts of the procedure. Duration of crying and the duration of the entire procedure, the patient's level of pain and anxiety, the caretaker's level of anxiety, need for restraining devices (papoose) and the efficiency of the procedure will be measured. In order to perform the survey and self-assessment procedures, we will implement the use of measurement scales including a novel "emoji" child distress assessment scale, and a published adult anxiety scale (State-Trait Anxiety Inventory Form Y-1). The data will be analyzed using descriptive statistics.

Using Hypnosis and Virtual Reality During Pre and Postoperative Cardiovascular Surgery.

Nowadays, the use of cognitive behavioral therapies has become quite common in the clinical care. Different non-pharmacological techniques, including hypnosis and virtual reality are currently used as complementary tools in the treatment of acute and chronic pain (Pourmand et al., 2017; Vanhaudenhuyse et al., 2009). A new technique called 'virtual reality hypnosis' (VRH) (Patterson et al., 2004), which encompasses a combination of both tools, is regularly used although its actual function remains unknown to this date. With the goal to improve our understanding of VRH combination effects, it is necessary to elaborate randomized and controlled research studies in order to understand their actual function in individual's perception. 100 patients who are undergoing a cardiovascular surgery at the Liège University Hospital will be randomly assigned to four conditions (control, hypnosis, VR and VRH). Each participant will receive two sessions of one of the techniques: one the day before the surgery and one other session the day after, in intensive care units. Physiological parameters will be taken and participants will fill in a questionnaire which evaluates their level of perceived immersion, their level of anxiety, fatigue, pain and relaxation. A short interview will also be conducted to give participants the opportunity to openly describe their experience. This study will help to expand the knowledge regarding the influence of these techniques on patient's cognition, perception and sensation .

Cell Phone-supported Cognitive Behavioural Therapy

Definitions CBT-ubiquitous - Cognitive behavioural therapy (CBT) provided with all-time (ubiquitous) support by cell phone applications CBT-TAU - Cognitive behavioural Therapy provided 'As Usual' CBT-placebo - Cognitive behavioural therapy provided with access to a digital audio player with self-administered materials for stress management and relaxation. Study objectives to compare the relative effectiveness of a computerized ubiquitous CBT (CBT-ubiquitous) against anxiety disorders with CBT-treatment as usual (CBT-TAU); to assess the safety and tolerability of the CBT-ubiquitous as compared with CBT-TAU and CBT provided with access to a placebo technical device (CBT-placebo).. Study implementation The investigators hypothesize that all active treatments are superior to placebo given evidence that face-to-face CBT is effective in the treatment of both anxiety and depression. The investigators also predict that patients receiving CBT-ubiquitous will show greater improvement than those given CBT-TAU, and assume that CBT-ubiquitous leads to improved adherence with treatment compared to CBT-TAU and CBT-placebo. The investigators will use an intention to treat (ITT) approach to fulfill the study aims, by which the investigators mean that clients are analysed as randomized rather than by treatment actually received. The study will be implemented in the central district of Östergötland County Council (pop 145.000). The research group has in two previous projects (VINNOVA 2005-2007, VINNOVA 2008-2009) established cooperation with the 'Unga Vuxna' (Young Adults) service provided in this district. The service supplies CBT to the subpopulation experiencing early stages of mental disorders in the age group 16-25 years (n=20,000) in order to prevent development of significant psychiatric disease and disability. The service employs six therapists with at least one year specific CBT training and is co-located with Primary Healthcare Centres (PHCs) in downtown Linköping, Åtvidaberg and Kisa. Clients staying outside the urban areas can make appointments with therapists at their closest PHC on a weekly basis.

Indeed, anxiety in relation to infertility happens frequently and over time, can become stressful for our patients. This level of stress influences the effect of the infertility treatment. Reducing anxiety levels could promote the ability of the patients to face this stress and promote a greater chance of pregnancy in that context.

The Effect of Breath Therapy and Emotional Freedom Technique on Public Speaking Anxiety

Introduction Public Speaking Anxiety (PSA), also referred to as public speaking anxiety, is a costly (Lépine, 2002) and disabling fear (Blöte et al., 2009), with prevalence rates ranging from 21 (Pollard and Henderson, 1988) to 33% (Stein et al., 1996; D'El Rey and Pacini, 2005) in community samples. FoPS has been reported as the single most commonly feared situation in both university and community samples (Pollard and Henderson, 1988; Holt et al., 1992; Stein et al., 1996; Tillfors and Furmark, 2007).EFT is an energy psychotherapy derivative application consisting of cognitive and somatic components used to improve personal negative emotions and related emotional and physical ailments.It is reported that the most recognized and used EFT among Energy Therapies is an effective method especially on anxiety, depression, burnout, stress management, and fears (Feinstein 2008; Irmak Vural & Aslan, 2018; Sezgin, 2017; Sezgin & Özcan, 2009).Breatihin therapy is another one. Purpose of the study The current study aims to determine the effect of the Breath Therapy and Emotional Freedom Technique on public speaking anxiety in Turkish Nursing Students. The purpose of the study was to evaluate two research hypotheses as follows: H1: Emotional freedom technique is effective in reducing public speaking anxiety H2: Breath Therapy is effective in reducing public speaking anxiety A randomized controlled experimental research design will be used. The study complied with the guidelines of the Consolidated Standards of Reporting Trials (CONSORT) checklist. The data will be collected with demographic information form, Subjective Discomfort Unit Scale, The State-Trait Anxiety Inventory, and Speech Anxiety Scale.demographic information form: This form contains 5 questions including the age, gender, economic status, place of residence of students in the form developed by the researchers. This scale, which is used in energy therapies and evaluates the individual's own discomfort, is scored between 0 and 10. 0 no discomfort 10 is considered to be unbearable discomfort (Wolpe, 1973). The individual should evaluate the discomfort he feels at that moment and give a score. This score serves as a concrete and basic starting point for the patient's condition at the time of application and reflects the change at the end of the application. According to the Church; EFT; Individuals' discomfort assessments are made using the Subjective Units of Disturbance (SUD) scale used by Wolpe (Church, 2013a). he State-Trait Anxiety Inventory (STAI Tx-1 and Tx-2) It was developed by Spilberg et al to detect the state and trait anxiety levels of individuals. Its validity and reliability in Turkish was done by Öner and Lecompte. The scale consists of a total of 40 questions, 20 questions of state anxiety and 20 questions of constant anxiety. State anxiety scale; to state how the person feels at any time and under any circumstances; It is asked to answer taking into account the feeling related to the situation. On the scale of trait anxiety; one is often expected to interpret how he feels. There are positive and negative expressions on the scale. These statements; are expressed as plain and inverted expressions. Straight expressions, negative emotions; reversed statements reflect positive emotions. There are 8 reversed statements on the state anxiety scale and 7 on the trait anxiety scale. Items on the state anxiety scale 1, 2, 5, 8, 10, 11, 15, 16, 19, 20, and on the trait anxiety scale 1,6,7,10,13,16,19. items are reversed statements. The emotions, thoughts and behaviors stated in the state anxiety scale are answered as 1: none, 4: completely according to the violence level of the experiences. Feelings, thoughts and behaviors stated in the trait anxiety scale are also marked as 1-almost never, 4-almost always according to the frequency level. For both scales; 0-19 points (none), 20-39 points (mild), 40-59 points (moderate), 60- 79 points (severe), 80 points and above are considered as very severe anxiety. Cronbach alpha value of the state anxiety scale was 0.93; The Cronbach alpha value of the trait anxiety scale is 0.94 (Öner, 1998). In the present study, Cronbach's alpha value was found to be 0.81 for the STAI Tx-1 and 0.76 for the STAI Tx-2. Speech Anxiety Scale (CUR): It was developed by Yaman and Sofu (2013) to evaluate students' speech anxiety levels. The scale consisting of 25 items is scored as 5-point Likert type and 1: strongly disagree, 4: strongly agree. In the scale, items 5, 6, 9, 10, 13 and 14 are reversed. A high score indicates that the person's anxiety to speak is high. Cronbach alpha value of the scale was found to be 0.79. In this study, Cronbach alpha value is 0.78.

Medical Hypnosis Against Anxiety and Pain in Electro-neuro-myography

Electroneuromyography (ENMG) is a regular neurological investigation that might be painful or unpleasant. Medical hypnosis can avoid these sensations, and is more and more performed during other medico-technical procedures (radiology, biopsies for example). This study aims at evaluate the effect of medical hypnosis on pain and anxiety during ENMG, in patients presenting with anxiety. Our hypothesis is that the anxiety and pain during ENMG will be lower in patients with hypnosis compared to patients with standard care. This will allow us to help patients with this tool during most ENMG procedures, and extend it to other electrophysiological procedures in the future.
