Does Caffeine Reduce Postoperative Bowel Paralysis After Elective Colectomy?

Postoperative bowel paralysis is common after abdominal operations, including colectomy. As a result, hospitalization may be prolonged leading to increased cost. A recent randomized controlled trial from the University of Heidelberg showed that consumption of regular black coffee after colectomy is safe and associated with a significantly faster resumption of intestinal motility (Müller 2012). The mechanism how coffee stimulates intestinal motility is unknown but caffeine seems to be the most likely stimulating agent. Thus, this trial addresses the question: Does caffeine reduce postoperative bowel paralysis after elective laparoscopic colectomy? Patients after laparoscopic colectomy will receive either 100 mg caffeine, 200 mg caffeine, or 250mg corn starch (placebo) 3 times daily in identically looking gelatin capsules. The study is a randomized, controlled trial, with blinding of physicians, patients and nursing stuff (evaluating the endpoints). Primary endpoint will be the time to first bowel movement.

Cerebral palsy (CP) is defined as a disorder of the developing brain that causes movement disorders and may be associated with other neurologically based disorders. Gait abnormalities are a direct result of damage to the motor areas of the brain and include symptoms such as spasticity, dystonia, weakness, loss of selective muscle control, dependence on primitive reflexes, abnormal muscle and inadequate balance reactions. Walking backwards during activities of daily living is as important as walking forward. Some of these activities are stepping back towards the chair, stepping back when opening the door and pulling the door, reflexively leaning back when suddenly encountering an obstacle or uneven ground. In addition, backward walking is defined as a more complex activity that requires more neuromuscular control, proprioception sense, and protective reflex activation than forward walking. Selective motor control is an essential part of typical human movement, allowing for smooth and discrete control of joint movement. Impaired selective motor control causes abnormal reciprocal muscle activations or involuntary combined movements, leading to difficulties with coordination, balance, walking efficiency, and symmetry. Impaired selective motor control is associated with poor gross motor function and balance control, severe general gait deviations, and decreased walking speed. The aim of this study is to examine the relationship between forward and backward walking and selective motor control, trunk control and balance in children with cerebral palsy.

Brachial Obstetrical Palsy

POPB is a consequence of the stretching of the nerve roots (C5, C6 + / C7, C8 or T1) of the brachial plexus at birth. One third of patients will have sequelae. The most common is the appearance of a deficit of passive and active mobilities in the movements in external rotation of the shoulder especially in external rotation (RE) elbow to the body, despite daily rehabilitation. At present, this stiffness is attributed to an imbalance between the external rotator muscles (mainly infraspinatus) that would be atrophied and the internal rotator muscles (subscapularis, pectoralis major, latissimus dorsi) that would be slightly affected In case of no or negative RE from the age of 1 year, there is a surgical indication to operate these children. At present, surgery to lift internal retractions is the only therapy used, but despite this surgery and intensive rehabilitation, in some patients mobility deficits re-occur in a few years. Thus, some teams systematically perform a muscle transfer to strengthen the outer rotator muscles deficit during the initial operation. Other teams (of which principal investigator is part) do this transfer only secondarily and in some patients. Investigators lack objective and scientific criteria for the indication of this second muscle transfer surgery and the etiology of retractions is not clearly defined. In humans, subscapularis is innervated by the C5 and C6 roots, which are constantly affected in POPB. It can be assumed that subscapularis may have an atrophy in POPB patients. To date, no anatomopathological study has been performed on the internal / external rotator muscles of patients with POPB that can give indications on recurrences. Based on our clinical observations and literature data, the main hypothese is there is amyotrophy of subscapularis and / or infraspinatus in POPB patients with shoulder stiffness.

Effect of Adjuvant Hyperbaric Oxygen Therapy on Bells Palsy Outcome

This is a randomized, single blinded, non-placebo controlled that will compare one group of Bells Palsy patients receiving the current standard of care including oral corticosteroids and oral antivirals against an experimental group receiving the current standard of care in addition to hyperbaric oxygen therapy. Outcome assessment will be based on both objective analyses of facial movements as well as subjective quality of life scales.

In 2022, the prevalence of Cerebral Palsy (CP) is 2.5 cases per 1000 live births. In France, 125,000 persons are affected by CP. Many treatments and therapies exist to reduce CP-related disorders, but CP remains uncurable. Motor Rehabilitation Courses (MRC) are one of those therapies used with children with CP. Their uses has not been studied in France yet. The aim of this survey is to find out about the use of motor rehabilitation courses for children with CP and, where appropriate, the content of these courses. This questionnaire will be used to determine the use and different types of MRS in which children participate.

The Brainway Deep Transcranial magnetic stimulation (DTMS) is a new technology of TMS using a particular coil, i.e. H-coil, able to stimulate deeper regions of the brain. Only few studies in literature have evaluated the efficacy of DTMS in Parkinson's Disease and parkinsonism; in particular in PSP patients, a case report showed an improvement in language.

Interscalene Block Versus Supraclavicular Block for Shoulder Surgery

Interscalene brachial plexus block constitutes the analgesic criterion standard for shoulder surgery. However it is associated with a high incidence of hemidiaphragmatic paralysis (HDP) that may not be tolerated by patients with chronic pulmonary disease. This randomized controlled trial will compare ultrasound-guided interscalene block (ISB) and supraclavicular block in patients undergoing arthroscopic shoulder surgery. The main outcome is static pain at 30 minutes after arrival in the post anesthesia care unit (PACU) as measured by a numerical rate scale (NRS) from 0 to 10. Our research hypothesis is that interscalene and supraclavicular blocks will result in equivalent postoperative analgesia at 30 minutes in the PACU. The equivalence margin is set at 2 points
