Long-term Effects of the Addition of Diathermy by Emission of Radiofrequency to Therapeutic Exercise...

Introduction: the management of Patellofemoral Pain (PFP) is focused on therapeutic exercise (TE) to improve muscle strength and motor control. Recent studies suggest that the addition of radiofrequency diathermy (RFD) obtain greater short-term improvements in knee pain than TE alone. As there is no follow up data, the aim of this research is to assess the long-term effects of adding RFD to TE on pain, function and quality of life on PFP patients. Methods: a single-blind randomized controlled trial will be conducted. Participants diagnosed of PFP will be allocated in either a TE group or a RFD+TE one. Sociodemographic data, knee pain, and lower limb function will be collected. Each group will performed 20 min of daily knee and hip supervised TE along three weeks with the addition of ten sessions of RFD for the RFD+TE group.

Rotational deformities, such as femoral (thigh bone) and tibial (leg bone) torsion, are established causes of patellofemoral (kneecap joint) pain and instability. Currently, computerized tomography (CT) remains the gold standard assessment tool for measuring the degree of rotational deformity. However, there is disagreement as to the thresholds for surgical correction as it is suggested that some individuals compensate for the deformity better than others. Gait (walking) analysis provides important information of the biomechanical parameters undertaken in the knee joint during dynamic movement. In patients with rotational deformities, several variations from normal gait patterns have previously been described by several authors. Therefore, it is questioned whether these parameters can be correlated with the static features of CT scans, in individuals with rotational deformities. This study aims to determine whether gait analysis is a useful tool in the diagnosis and treatment pathway for rotational abnormalities in patellofemoral pain or instability. A secondary aim of this study will be to assess whether gait analysis can detect a rotational deformity in the lower limb and determine whether the changes seen are proportionate to the level of deformity. Patients to be included in this retrospective study are, adult patients, with presumed rotational deformity of the lower limb, with anterior knee pain, who have been referred to the gait laboratory for assessment, and for CT rotational profiles. Only patients who have given consent to the gait laboratory for their information to be utilised in research will be included. Two researchers will record measures of rotational deformity from the CT images. The gait laboratory assessment includes measurements of biomechanical parameters based on joint movement through the gait cycle - these parameters will be assessed and compared with CT measures for correlation. All research will be conducted at the hospital where the gait laboratory assessment has been performed.

The aim of the study is to evaluate the myofascial chain lines in addition to the general evaluation parameters in patients with patellofemoral pain syndrome (PFPS) and to determine whether deviations from normal in the myofascial structure have an effect on the development of PFPS. There will be two groups in this study. One of the groups will consist of 28 patients aged between 25-50 years. The other group will consist of 28 healthy individuals between the ages of 25-50 years. Postural problems that can be seen in the superficial anterior myofascial chain line will be evaluated. These problems: forward head, q-angle, genu varum, genu valgum, foot postural problems (pes planus, pes kavus vb.) and pelvic tilt. In addition to all these assessments, knee pain and the biomechanical properties of the anterior myofascial tissue will be evaluated.

The aim of the study will be to assess the effectiveness of Diacutaneous Fibrolysis technique on pain, range of motion (Patellofemoral joint mobility test and muscle length test), strength and function in patients with Patellofemoral Pain Syndrome and muscle shortening. Muscle imbalance is one of most important factor associated with Patellofemoral Pain Syndrome. Shortening of lateral muscles of the thigh have a close relationship with patella position and tracking and patellofemoral pain. The scientific evidence for effectiveness of any soft tissue mobilization technique in patellofemoral pain syndrome patients is poor. Diacutaneous Fibrolysis is a non-invasive physiotherapeutic technique to release adherences and fibrosis between the different musculoskeletal structures. Diacutaneous Fibrolysis is applied by a set of metallic hooks having the advantage of allowing a deeper and more precise application, which could not be achieved manually. The present study evaluates the effectiveness of Diacutaneous Fibrolysis technique applied in the soft tissues more related with patellofemoral pain.

The Effect of Kinesio Taping® on Motor Control in Patients With Patellofemoral Pain Syndrome

Background: Recently, several professionals are using the Kinesio Taping® in the treatment of pathologies related to the knee, aiming to improve the motor control, increase the response of muscle activation and promoting better joint stability. Purpose: The objective of this study is to evaluate the effect of the application of Kinesio Taping® in motor control of patients with patellofemoral pain syndrome. Methods: This is a 2-arm, randomized with a blinded assessor. We selected 40 women with patellofemoral pain (age 25.2±4,years) were randomly assigned to one of two groups: placebo group (n= 20) and the Kinesio Taping® group (n=20). The placebo group received the application of a non-stretchable tape (Micropore) crosswise on the thigh and the Kinesio Taping® group followed all the criteria of the method in placing the bandage. All patients received the evaluation of the motor control banding pre and post a patch, by means of the method Y balance test.

This study primarily aims to investigate arthrogenic muscle inhibition (AMI), an underlying mechanism in patellofemoral pain (PFP), which remain to date understudied. Although, studies have revealed that AMI needs to be eliminated before improvements in muscle strength can be achieved, to date quadriceps muscle inhibition in patients with PFP remains understudied. To date the influence of AMI on functional performance and the direct link to pain in patients with PFP remain unclear. This study aims to investigate the link between AMI to pain, functional performance and how an acute treatment can affect functional performance and pain.

The Effects and Predictors of Patella Taping in the Treatment of Patellafemoral Pain Syndrome

Background: Taping has been used for more than 20 years for patellofemoral pain syndrome (PFPS) but the effectiveness is still controversial. Purpose: This prospective study was conducted to investigate the effect and predictors of effectiveness of taping in treating PFPS. Study design: Prospective cohort study Methods: One hundred consecutive patients with the diagnosis of PFPS were included in the study. Factors including sex, age, body mass index (BMI), Q angle, lateral patella displacement (LPD), lateral patellofemoral angle (LPA) and pre-taping pain score were measured. One well-trained therapist applied adhesive tape to each patient by McConnell approach. Before and after taping, patients completed a visual analog pain scale (VAS) after performing a step-down from an 8-inch platform. Paired-t test was used for the difference of the VAS score measured before and after taping. Patients with improvement of more than 1 point in VAS score after taping were considered responsible, and others were non-responsible. The results were analyzed by logistic regression.
