Peri-ImplantitisPeri-implantitis is defined as the pathological condition around dental implants characterized by inflammation in the peri-implant mucosa and progressive bone loss, eventually leading to implant loss. Peri-implantitis is thought to be a disease analogous to periodontitis with a prevalence reaching 22%. Though peri-implantitis is readily recognized as a part of modern dentistry, the exact etiology or an effective treatment regimen hasn't been established yet. Thus, contemporary research is orientating toward acknowledging the aetiologic and risk factors of the disease and of course establishing prognostic markers for disease prevention. Microbiota residing in the subgingival plaque are considered the main etiologic factor of the disease, however, current literature has not concluded on the exact microbial composition of peri-implant lesions. In addition, genetic predisposition has been recognized as a risk factor for disease initiation and progression and several observational studies have addressed the potential association between various gene polymorphisms and the occurrence of peri-implantitis. Lastly, to establish effective preventive measures, several biomarkers have been evaluated as potential diagnostic and prognostic markers of disease progression. Objectives: To identify the relationship of peri-implantitis with Cycloxygenase-2 (COX-2) and MMP-8 gene polymorphisms. Cyclooxygenase catalyzes the production of prostaglandins (PGs) which are an important inflammatory mediator participating in the pathogenesis of peri-implantitis. In addition, PGE2 expression in the peri-implant crevicular fluid will be assessed. To characterize the microbiota associated with peri-implantitis lesions, using novel identification techniques enabling the identification of specific opportunistic bacteria associated with the disease. To test the diagnostic accuracy of a modern chairside test, using metalloproteinase-8 (MMP-8), an enzyme implicated in the pathogenesis of the disease, as a biomarker of disease progression.

The goals of peri-implantitis treatment are to resolve inflammation and to arrest the progression of disease. It is important to systematically gather information on the effect of surgical peri-implant treatment and to assess different protocols regarding chair-side maintenance of peri-implant tissue after surgery The aims of this clinical investigation are to evaluate the clinical, microbiological and radiographic outcomes of surgical treatment of peri-implantitis and to evaluate the efficacy of 2 supportive treatment protocols based on the use of titanium cyrettes or by the use of a flexible, biodegradable chitosan brush. Furthermore, to evaluate the impact of this therapy on selected biochemical markers associated with chronic inflammation and bone tissue destruction.
Metronidazole as an Adjunct of Non- Surgical Treatment of Peri-implantitis
Peri-ImplantitisThe use of systemic antibiotics as and adjunct to non-surgical peri-implant therapy may be an improvement in comparison to these therapies alone. The primary objective is the evaluation of significant changes in probing pocket depth between non-surgical with or without antibiotics. This is a controlled-placebo clinical trial design. Patients with osseointegrated oral implants will be selected and recruited from a university clinic. Oral hygiene instruction and non-surgical debridement at implants will be provided with ultrasonic devices and immediately after, patients will be prescribed: Group Control: A placebo with the same characteristics as the antibiotic Group Test: Systemic antibiotics (Metronidazole 250mg, 2 capsules three times a day, for 7 days Three and six months after non-surgical treatment, clinical parameters will be registered and radiographs compared using reproducible landmarks. Any adverse event will be also recorded.

Peri-ImplantitisPeri-implantitis is a pathological condition occurring in tissues around dental implants, characterized by inflammation in the peri-implant connective tissue and progressive loss of supporting bone. The goals of peri-implantitis treatment is the resolution of peri-implant soft tissue inflammation and stabilization of the bony attachment (e.g., the level of osseointegration). For this decontamination of the implant surface is mandatory. In order to increase implant surface decontamination, several adjunctive tools have been proposed and investigated both in pre-clinical and clinical studies such as the use of photodynamic therapy and lasers. So far, no data are available to clearly demonstrate the efficacy of the adjunctive use of a diode laser in the non-surgical treatment of peri-implantitis. Therefore, the aim of the present randomized controlled trial (RCT) is to investigate the adjunctive effect of the application of a diode laser to treat peri-implantitis lesions by means of a non-surgical approach. A total of 30 patients is randomly allocated to two groups. The test group receives 3 x nonsurgical mechanical treatment with diode laser application whereas the control group receives the same treatment with sham laser application. The primary outcome is the peri-implant pocket probing depth at 12 months.
Ozone Therapy as an Adjunct to the Surgical Treatment of Peri-implantitis
Peri-ImplantitisDecontamination procedure is a challenging factor that affects the success of surgical regenerative therapy (SRT) of peri-implantitis. The purpose of the present study was to determine the impact of additional ozone therapy for the decontamination of implant surfaces in SRT of peri-implantitis. A total of 21 patients with moderate or advanced peri-implantitis were randomly allocated to the test group (ozone group) with the use of sterile saline with additional ozone therapy or the control group with sterile saline alone for decontamination of the implant surfaces in SRT of peri-implantitis. Clinical and radiographic outcomes were evaluated at baseline and 6 months postoperatively
