Biotherapy present specifics risks that patients must know and learn to manage. A national survey has been carried in this study to evaluate patients safety skills. (wording: " cross study of safety skills of 677 patients treat by biopharmaceuticals for an inflammatory rheumatism). This survey has allowed showcasing patients difficulties in managing their treatment, including for those under subcutaneous biotherapy. More than 60% patients interviewed doesn't know symptoms to bring them to consult and mainly postpone to their injection. The aim of this study is to prove that nursing consultation can allow the patient to become independent in treatment management and thus avoid occurrence of adverse event.

Expanding Exercise Programming for Veterans Through Telehealth

Arthritis

The goal of this study is to evaluate the acceptability and feasibility of a group telehealth exercise program for Veterans with lower extremity osteoarthritis (OA). Methods: This is a randomized pilot trial where Veterans (n=50) receive a group telehealth exercise program (3 months), followed by an exercise maintenance phase (6 months) that includes either text messaging alone or text messaging plus group telehealth exercise booster sessions. The primary outcomes for this study are feasibility and acceptability of the intervention. Feasibility will be assessed through number and source of referrals, recruitment, and retention. Acceptability will be assessed through surveys and interviews for participants and providers addressing reasons for participation, satisfaction, perceived appropriateness, and overall experience containing Likert scales and both yes/no and open-ended questions. Exploratory outcomes, assessed at multiple time points during the course of the study, will measure changes in the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and changes in performance measure and physical activity assessed remotely through individual telehealth sessions. Data will be collected at baseline, 3-, 6-, and 9-month follow-up.

Stronger Together Psoriatic Arthritis Wellness Study

Psoriatic Arthritis

Patients diagnosed with psoriatic arthritis (PsA) confront decisions about how to treat their disease, interact with their health care providers, and modify lifestyle choices that may improve treatment outcomes. With sponsorship support from Janssen and in partnership with the Cleveland Clinic, this survey study will seek to understand and examine whether people living with PsA are interested in and motivated to participate in wellness activities. This cross-sectional observational survey study will help researchers and clinicians to better understand what some of the barriers and facilitators experienced by patients are when considering participation in guided online wellness activities. People that qualify for the study and complete a 10-minute survey will be invited to participate in Immune Strength, an online wellness program developed by the Cleveland Clinic. Immune Strength is a free 10-week program for creating better habits for a more resilient immune system. By following this program, participants will have the opportunity to learn about how to develop strategies and implement behavior changes for healthier immune functioning. Participants will be recruited by the Global Healthy Living Foundation (GHLF). GHLF is the parent organization of the CreakyJoints® (CJ) arthritis patient community and primary site for the PCORI-funded Arthritis Patient Partnership with Comparative Effectiveness Researchers (AR-PoWER) Patient Powered Research Network (PPRN), known as ArthritisPower®. ArthritisPower is a subset of CreakyJoints members who have expressed interest in participating in research and signed an informed consent form to participate in the PPRN and its associated research. Any individuals recruited from CreakyJoints or Facebook, who are not already members of ArthritisPower will be encouraged to join the ArthritisPower registry research app, but will not be required to do so in order to participate in the study.

The prevalence of Sjogren's syndrome (SS) in rheumatoid arthritis (RA) patients varies from 3.5 to 31%. Between 30% and 90% of patients with (RA) have dry eye and/or mouth syndrome. To date, no studies have assessed whether RA patients have echostructural changes in their salivary glands suggestive of SS and the factors associated with these changes.The aim of this study is to investigate if there are changes in the echostructure of the salivary glands of RA patients, especially in patients with dry syndrome.

Gut Microbiota: a Player in Chronic Pain in Patients With Rheumatoid Arthritis?

Although rheumatoid arthritis (RA) treatments are very effective in reducing inflammation, chronic pain persists in 20 to 30% of patients. The intestinal microbiota can participate on the pain pathways and a decrease in Faecalibacterium has been associated with chronic pain and chronic fatigue. RA patients have an altered gut microbiota or dysbiosis. Among the bacteria that are most often differentially represented between RA and control, Faecalibacterium, was also found less abundant in RA patients. The composition of the gut microbiota has never been evaluated in relation to the clinical phenotype of RA patients and in particular to the presence of a diffuse pain. In this study, investigators will test whether the gut microbiota of RA patients, and in particular the decrease of Faecalibacterium, would promote pain sensitization phenomena, and thus, chronic pain despite the control of joint inflammation.

Use of Allograft Adipose Matrix for Small Joint Arthritis of the Hand

Osteoarthritis Hand

The purpose the research is to evaluate the safety and efficacy of injection of adipose allograft matrix (AAM) to the small joints of the hand for treatment of early stage osteoarthritis. The hypothesis is that use of AAM injected directly into the joint will show improvements in pain and disability while providing a safe, off-the-shelf alternative which can obviate the need for, and risks associated with, current treatment options with autologous fat transfer. As standard of care, routine strength, pain scale scores (VAS) and range of motion will be recorded, a baseline disability survey (DASH score) will also be administered. After these have all been recorded and administered in a separate visit the patient will undergo the lipofilling procedure. The subject population will include patients over the age of 18 who present with joint pain of the hand with radiographic evidence of osteoarthritis.

Efficacy and Safety of Iguratimod in Patients With Hand Osteoarthritis (ESIGO)

Hand osteoarthritis is one of the most common arthritis, resulting in pain in finger and thumb base joints. The disease is characterized by pain and stiffness of the affected joints and is the most common cause of disability in aged people. Currently limited therapy options are available. Synovial inflammation is involved in the joint pain. Iguratimod is a small disease-modifying compound that can influence anti-inflammatory pathways in models of rheumatoid arthritis. It has an anabolic effect on the bone metabolism of infected joint by osteoclastogenesis inhibition and osteoblast differentiation. The investigators hypothesize that Iguratimod will alleviate pain of patient with inflammatory hand osteoarthritis, and that a beneficial effect of Iguratimod on pain will be accompanied by a decrease of synovial inflammation.
