Evaluating A Drug Testing and Graduated Sanctions Program in Delaware

The objective of this three year project funded by the National Institute of Justice is to conduct a program evaluation of the "Decide Your Time" program at the Hares Corner Office of the Delaware Department of Probation and Parole. The program is intended to provide an alternative to incarceration by monitoring chronic drug offenders through increased, regularly scheduled, known urinalysis testing, coupled with increasing sanctions and referral to treatment for positive tests. The program was designed and is being implemented by the Delaware Department of Probation and Parole. Offenders will become eligible for the program when they test positive for drugs at their initial probation intake at the Hare's Corner Probation Office in New Castle, Delaware or are moved from standard probation to intensive supervision due to failed urine tests. Due to budgetary constraints, the office lacks sufficient resources to place all eligible offenders into the program. The Department of Probation thus intends to randomly assign eligible offenders to either intensive supervision probation (ISP)(n=400) or the Decide Your Time program(n=400). Those in ISP will receive the normal intensive supervision requirements, including weekly visits and random urinalysis. Those in the enhanced condition will receive regularly scheduled urinalysis coupled with referral to treatment, if required, and a program of graduated sanctions. The Center for Drug and Alcohol Studies is conducting an evaluation of the program that involves examining data on effectiveness, and collecting and examining data on program implementation, and probation officer and client perspectives of how the program worked. It is hypothesized that those randomized to the Decide Your Time program will have fewer arrests, violations and positive urine screens upon program completion than those randomized to the control condition.

The relationship between cytomegalovirus infection and recurrence of hepatitis C in liver transplant recipients remains controversial. Although some studies (Teixeira et al., 2000; Singh et al., 2005)have not found an association between recurrence of hepatitis C and CMV infection, studies such as Rosen et al. show that 50% of patients with CMV infection suffered cirrhosis durig follow-up period, while between not-infected patients the rate was 11%. To clarify this question, a non-interventional study will be carried out in order to assess if CMV replication is a risk factor for graft dysfunction in liver transplant recipients.

Distant metastasis of cancer remains the major cause of cancer death. One of the evidence is that some rare cells shed from primary tumor exist in the circulation of cancer patients, which has been proven to be related to cancer relapse and distant metastasis. The number of circulating tumor cells (CTCs) or the expression status of specific marker(s) on them also correlated with the disease prognosis and treatment effects, which might change the decision of treatments. In recent years, as specific disruptive genes were discovered, such as epidermal growth factor receptor (EGFR) in non-small cell lung cancer,Kirsten rat sarcoma (KRAS) in colorectal cancer, the response rate to treatment, disease control and survival have been much improved. However, the molecular information obtained from cancer tissue depends on repeated biopsies, which is very risky and invasive to cancer patients. By means of the advances of CTCs sampling technique with genetic analysis, repeated follow-up for specific gene profiles is possible. However, the protocol has not been well-established and mature, even the correlation between primary cancer tissue and CTCs remains unknown. To tackle the problems above, the aims of the project is to isolate high-purity CTCs by the optically induced dielectrophoresis (ODEP)-based device or other cell sorting techniques and transfer to next-generation sequencing (NGS) analysis for specific disruptive genes. In the first year of the project, the investigator will testify and stabilize the platform utilizing healthy donors' blood and cancer cell lines and adjust the detailed experiment conditions. In the following year, the investigator will enroll newly diagnosed metastatic cancer patients with the disruptive gene mutation(s) and follow up the events under gene-based therapy. Comparison of NGS information between cancer tissue and CTCs will be also made as one of the major endpoints. In brief, the investigator expect the study could establish a practical method to get genetic information, to reduce the risk of re-biopsy and to achieve the ultimate goal of precision medicine.
