Diagnostic Ultrasonography in Physiotherapy

Ankle injuries are among the most common traumatological injuries of the lower limb accounting for approximately 50% of all sports injuries and 25% of musculoskeletal injuries in general. Correct initial diagnosis and proper management is important to reduce the risk of recurrent ankle instability and other complications, such as reduced range of motion, increased ligament laxity, instability, tendon enthesopathy, possible swelling, formation of calcifications, reduced load on the affected limbs while standing and walking, increased pain intensity and pain duration. To date, few studies showed that the effectiveness of physical therapy increases when it is modified based on diagnostic ultrasonography findings. Sonography is not burdensome for the patient, has high reproducibility, and enables dynamic examination and comparison of the interrelationships of individual structures. The aim of this study is to investigate if targeted physiotherapy for post-traumatic ankle conditions designed based of diagnostic ultrasonography findings is more effective than standard physiotherapy. The study hypothesis is that in the experimental group there will be a smaller number of complications of post-traumatic ankle conditions than in the control group. If confirmed the study could have clinical implications.

Ankle Sprains

Ankle sprains are the most common musculoskeletal complaint of children presenting to the emergency department (ED). Healing can often be protracted, leading to prolonged pain, missed school and work, and delayed return to a normal activity level. Smartphone apps have been shown to be associated with greater caregiver knowledge and improved outcomes in a number of conditions but have not been explored in ankle sprains. We would like to know if using a smartphone app for children with ankle inversion injuries leads to improved functional outcomes such as pain, mobility, and return to activity. We will be comparing a smartphone app that provides education and daily management reminders to a paper handout to see if the former leads to improved functional recovery.

Flossing Protocol and Manual Therapy in Young Subjects With Recurrent Ankle Sprains

The most frequent lateral ankle sprain mechanism consists of talocrural plantarflexion added to a subtalar inversion. The anterior talofibular ligament, in plantar flexion, suffers maximum stress, being very sensitive to injury mechanisms with inversion of the foot, being the cause of up to 70% of sprains. The risk of injury recurrence must be taken into account. In sports practice, this percentage of recurrence is especially relevant. Randomized clinical study. Participants will be randomized to a control or experimental group. The dependent variable will be the range of motion in ankle dorsiflexion. Secondary variables will be pressure pain threshold, and stability of the foot and ankle. Three evaluations will be carried out: before the start of the study (T0), after the intervention (T1) and after a 4-week follow-up period (T2). Although the athletes show proficiency in these tests and measurements, re-injuries occur when returning to sports. The causes of these re-injuries are mostly functional deficiencies such as inadequate neuromuscular control and stability. In the competition or sports environment, especially in team games, the athlete also shows cognitive performance, such as communication with teammates and following the game, which are included in the game setup, as well as the physical performance. Performing many tasks or performances at the same time divides the focus of attention on the activities performed, and if the person cannot adequately meet the attention demands, the quality of one or more of the tasks performed will deteriorate. As the level of expertise in the sport increases, the athlete tends to manage his posture, balance and movement with automatic postural control and can focus his attention on a new task. The concept of focus of attention has been evaluated from different perspectives over time. If it is examined in terms of direction; It is divided into two as the internal focus of attention, which is used by focusing on body movements during the performance of the person, and the external focus of attention, which is used by focusing on the effect of the movement during the performance of the person. As the investigators planned in this study, a second cognitive task assigned to the participant simultaneously during his or her physical performance acts as an external focus of attention, allowing movement control during performance to be carried out by unconscious or automatic processes. The investigators's aim; It is to examine the balance and functional hop tests that the investigator will apply in athletes by combining them with a simultaneous dual cognitive task that will reflect the field conditions more realistically. In the meantime, investigators think that with the sharing of our results with the literature, it can contribute to both the decision-making processes to return to sports after injury and preventive rehabilitation programs.
