Health PromotionOrganizationsBackground: In the field of health promotion, the 'setting approach' has gained increased attention over the last decades. Following this approach, organizations and governments are urged to invest in health and health promotion. However, the dissatisfaction with the broad definition of the term 'setting' has grown. As many health promotion programs are carried out in organizational settings, substantial work has been done on how to acknowledge organizational factors in designing and implementing health promotion programs. Organizational settings differ from others because they can also address their clients as beneficiaries of health promotion programs, also known as 'client-oriented health promotion'. Among the most prominent organizational settings for health promotion are schools and hospitals, and more recently, long-term care (LTC-) facilities. However, dissemination and implementation of client-oriented health promotion in organizational settings seem to be very challenging. Most approaches dealing with barriers to health promotion implementation in schools, hospitals, and LTC-facilities rarely reflect the specific organizational characteristics. This negligence constitutes both, a research gap in the setting approach and in the design of health promotion practice. Hence, the aim of this study is to explore professionals' (teachers', health professionals', care aids') views and attitudes that influence the implementation of client-oriented health promotion programs between schools, hospitals, and LTC-facilities in Austria. Methods: With that aim in mind, the investigators chose a comparative qualitative design. Semi-structured interviews will be conducted with 90 professionals (30 in schools, 30 in hospitals, and 30 in LTC-facilities). In addition, non-participant observations as well as an extensive document analysis in each setting will be undertaken. The data will be analyzed by thematic analysis. Comparisons within as well as between the organizational settings will be conducted using selected categories. Discussion: To date, this study is the first of its kind that compares results of individual semi-structured interviews between different organizational settings. This study investigates professionals' views and attitudes on facilitating and hindering factors of implementing client-oriented health promotion programs and thus will provide a solid basis for future research activities and evaluation studies in the field of health promotion implementation.

Follow-up of AKI in Neonates During Childhood Years
Neonatal Acute Kidney InjuryChildhood Chronic Kidney DiseaseThe purpose of this study is to learn more about how to identify signs of early chronic kidney diseases in children who were born prematurely with low birth weight (less than 3 ½ pounds). Researchers plan to compare the kidney function in children who experienced acute kidney injury (AKI) in the Neonatal Intensive Care Unit (NICU) with those who did not experience it. Evidence from several studies and our experience at UVA show that older children who experienced AKI while in the Pediatric Intensive Care Unit (PICU) have increased risk of developing early chronic kidney disease, and they also show early changes in the urine and blood that is consistent with early chronic kidney disease. In this study, the investigators hope to determine if any of these changes can be detected in early childhood, and if so, at what age we can start detecting these changes.

Multi-scale Modeling of Breast Conserving Therapy
Breast CancerThe proposed study is a single-center, prospective, pilot, observation study to tabulate and measure the changes that occur during treatment and healing for breast conserving therapy (BCT) in women with non-metastatic breast cancer for comparison to the multiscale model of breast lumpectomy and healing in order to identify targets for improving BCT. The study will be comprised of 12 de novo breast cancer patients with non-metastatic breast cancer undergoing BCT. Subjects will be enrolled in the study during the 3 weeks prior to receiving their BCT surgery and will be followed for six months after the BCT surgery. A total of 6 visits (not including the surgery) are planned.

Salivary Biomarkers for Non-small Cell Lung Cancer Detection
Lung CancerThe investigators plan to recruit patients for a prospective study in patients in need of evaluation for lung lesions suspicious for cancer. Saliva samples will be collected before diagnostic evaluation including biopsy with subsequent blinded examination of the salivary markers without knowledge of the disease status. This prospective recruitment with retrospective blinded evaluation or PRoBE design satisfies the highest standards recommended by the National Cancer Institute for biomarker development. This process limits the selection bias that can confound retrospective studies. As the primary endpoint, a pre-specified multi-marker panel will be evaluated based on the combination of sensitivity and specificity. In addition, seven pre-specified individual candidate mRNA cancer markers and six internal reference or "housekeeping" genes will be evaluated. The performance of new multi-marker panels will also be assessed and compared with the prior pre-specified model based on sensitivity and specificity combinations as well as the area under the receiver operating characteristic curve.
