
Expanded Access Study of Fenretinide Lym-X-Sorb Plus Ketoconazole in Neuroblastoma
Recurrent NeuroblastomaNeuroblastomaCurrently there is no known effective treatment for recurrent/resistant neuroblastoma. Fenretinide is an anticancer agent that may work differently than standard chemotherapy medicines. It may cause the buildup of wax-like substances in neuroblastoma cancer cells, called "ceramides" or other chemicals, called 'reactive oxygen species'. In laboratory studies it was found that if too much ceramide or reactive oxygen species build up in neuroblastoma cells, they may die. In addition, researchers are testing to see if a drug called ketoconazole, commonly used to treat fungus infections, can increase fenretinide levels in the body by interfering with the body's ability to break down fenretinide. This study is being done: 1) to allow patients with recurrent/refractory neuroblastoma patients who would otherwise not be able to access fenretinide/LXS oral powder for treatment to do so; 2) to further describe the side effects of fenretinide and ketoconazole when given by mouth for seven days every three weeks; 3) to determine if a patient's tumor gets smaller after treatment with fenretinide oral powder plus ketoconazole or fenretinide oral powder alone.

Neuroendocrine TumorRationale: Patients with neuroendocrine tumors (NET) have a rare disease. Due to treating patients with a neuroendocrine tumor in 'NET knowledge centers' patients often have to travel long distances for follow-up visits at the outpatient clinic. Patients whose medical condition allows videoconsultation could save time by replacing outpatient clinic visits through videoconsultation for receiving follow-up care. Therefore, in this study we aim to introduce videoconsultation as a alternative for follow-up outpatient clinic visits in NET patients. Objective: The primary objective is to assess if use of videoconsultation in follow-up care for NET patients is feasible. We hypothesize that videoconsultation is a suitable medium for providing follow-up care in NET patients. Secondary objectives are to explore the amount of time videoconsultation takes in comparison with outpatient clinic visits and the acceptability and satisfaction of physicians and patients with using videoconsultation in follow-up care. Study design: The present study is a single-centre prospective feasibility study. Study population: Adult NET patients under surveillance or treatment of the department Medical Oncology at the University Medical Centre Groningen (UMCG) whose medical condition allows videoconsultation will be invited to participate. Intervention: Patient who give informed consent will participate in the study. Participants will receive follow-up care through videoconsultation instead of conventional visits at the outpatient clinic. Main study parameters/endpoints: The main endpoint is the feasibility of videoconsultation for follow-up care in NET patients. We hypothesize that videoconsultation is a suitable medium for providing follow-up care.

The data generated in this study will come from surveys, interviews, review of medical records. Data from this study will be utilized to guide future clinical trials for children with an acute case of TM or AFM. Parents and school aged children will complete an online survey 7 banks of questions. Each bundle of survey topics have 7-10 questions. We will have both the parent and child complete a outcomes based survey within 6 months of diagnosis and invite to participate every 4 months until study end in 2024.

"Performance of HPV DNA Test in Presence of Co-infection With Common RTIs"
Cervical Inflammation and Human Papilloma Virus PerformanceThere is a strong causal association between persisting genital tract infection with Human Papilloma Virus (HPV) and gradual progression of infection to cervical (mouth of uterus) cancer.The robust study from India have shown that single round of HPV DNA screening test to significantly reduce the cervical cancer mortality. The above findings are very encouraging since it demonstrates that a simple and reliable HPV DNA test which is now available in low income countries has a potential to be accepted as primary screening test in future. The cross-sectional studies from developed countries from year 1999-2004 which focused to determine the test characteristics of HPV Hybrid Capture 2 test (HC2) to determine CIN2 and higher grade lesions, reported test sensitivity of 90% to 100% with a mean of 95%. Similarly the cross-sectional studies from developing countries from 1993 to 2003 which have focused on test characteristics of HPV DNA Hybrid capture 2 (HC2) test to determine CIN2 and higher grade lesions has shown a sensitivity in the range of 50% to 91% with an average sensitivity of 79%.Among the developing countries cross-sectional study from India which evaluated test reported sensitivity of only 68.2%. There is a statistical significant difference ( p value = 0.003) of sensitivity of HPV DNA HC2 test between the developed and developing countries to determine CIN2+ Lesions while there is no difference in the specificity. A study that was conducted in Chinese women to detected the prevalence of HPV genotype among women with mucopurulent cervicitis, healthy women and women with Invasive cancer, the author reported a 10% higher failure rates to extract HPV DNA in cases of mucopurulent cervicitis as compared to other two groups. So the study will explore if if the presence of untreated co-infections with STIs/RTIs (Sexually transmitted infections/Reproductive tract infections) resulting in cervical inflammation is somewhere interfering with low sensitivity of HC2 test due to presence of blood and excess mucus associated with the conditions leading to low sensitivity of the test in context to Indian Scenario.
