Patients' Experiences in a Medicalized Hotel for COVID-19 Acute Care Support

The 11th March 2020, the World Health Organization (WHO) declared the novel coronavirus Covid-19 as a pandemic. Urgent reorganization was required to give answers to the needs of this new illness. Since mid March, the number of patients admitted every day grew exponentially, and despite strengthening the Home Hospitalization service, resources were insufficient to care for such a number of people at home. The HH was called to transform a hotel into a medicalized healthcare space. Current health policy emphasizes on patient experience, as one of the key components of quality of care. Analyzing patients experience will provide knowledge about their authentic concerns or what they identify as real needs, how they perceive the care received and will allow finding out if it was a good alternative. Patients must be involved in decision-making about their heath situation and treatment in order to provide an integrated healthcare. Aims: To assess the patient experience during hospital admission in a Medicalized Hotel for COVID-19, from 8th March to 25th May, and variables that may influence these experiences. Study type: Observational and descriptive study with a cross-sectional design. The study population consisted of patients admitted in Medicalized Hotel who met inclusion criteria. Sample size: 517 patients. Main variable: Patient experience (collected by the Picker Patient Experience questionnaire, PPE-15, translated to Catalan and Spanish), that content four open questions analyzed in a qualitative way. Other variables: Socio-demographic and review clinical records.

In most countries, after the measures taken to reduce the effects of the epidemic and quarantine practices, a remote screening tool has emerged in order to screen the needs developing after COVID-19 and to make the necessary rehabilitation applications in time. Sivan et al. A comprehensive multi-system phone scan of the COVID-19 Yorkshire Rehabilitation Screening Tool (C19-YRS) has been developed by a multidisciplinary rehabilitation team. This study, which will be carried out using the C19-YRS tool and the 30-second sit-and-go test that evaluates functional capacity and mobility conditions, is expected to contribute to the clinical and literature since most of the studies on COVID-19 focused only on the acute and subacute periods. In addition, considering the pandemic conditions all over the world, it will be possible to benefit from the advantages of the tele-assessment application and by reaching more people, possible clinical problems/needs and whether there is a change functional capacities in individuals after COVID-19 will be identified.

Management of Patients Suspected of COVID 19 With Ultra Low Dose Thoracic Scanner

The year 2020 was marked by the emergence of a new respiratory disease: COVID19 related to an infection with the coronavirus SARS-CoV-2 (name given by the WHO on February 11, 2020). Initially appearing in China, in the province of Hubei, this epidemic has rapidly spread to be declared a global pandemic on March 12, 2020 by the WHO.Given the current context of the COVID-19 epidemic, strict hygiene measures have been taken in scanning rooms with systematic bio-cleaning. Strategies have been modified as a matter of urgency, with changes in the practices of electro radiology manipulators who work in "isolation" to avoid contamination. The number of scanners has increased exponentially following the curve of the epidemic, making it more and more difficult to systematically check the images before the patient leaves the room. To ensure sufficient image quality for interpretation of all scans, a complementary acquisition in ULD, with very low exposure, was systematically added to the acquisition protocol. The standard acquisition in LD, associated with this acquisition in ULD remain well below the diagnostic reference thresholds dictated by the nuclear safety authority (NRD: 350 mGy.cm. In Nîmes: LD: 100 to 150 mGy.cm and ULD: < 15 mGy.cm). These two acquisitions allow us to avoid breathing and movement artifacts, etc., without having to have the patients return for a new scan in case of a bad acquisition. This avoids an overexposure of the staff to the risk of infection by the SARS-CoV-2 virus. The main objective of this retrospective study is to evaluate the diagnostic performance of ULD vs. LD for the accurate diagnosis of COVID-19 pneumopathy which presents a particular ground glass pattern Our study will demonstrate that the ULD scanner can be used in the search for COVID-19 pneumopathy and thus limit the exposure of patients to X-rays, especially since thoracic scans are often repeated.

Patients with severe hypoxemic respiratory failure due to Covid-19 are often treated with oxygen delivered through a high-flow nasal cannula (HFNC). This is according to guidelines. We have made the clinical observation that oxygenation sometimes improves when a mask (e.g. oxygen mask or inhalation mask) is applied on top of the HFNC. This has quickly become a clinical routine at intermediary care units at our hospital, where patients with HFNC are offered to test the intervention (mask + HFNC) as part of clinical routine. This study aims to evaluate this new routine in a standardised way.

End Stage Kidney Disease

Patients on haemodialysis are at higher risk of getting a severe form of COVID-19 if they become infected. Vaccinations are soon to arrive and offer great hope of controlling the current pandemic. It is likely that patients on haemodialysis will be amongst the first people to be offered vaccination against COVID-19 when they become available. While any vaccines offered to these patients will be safe to receive, the effectiveness of the vaccines at giving immunity to being infected with COVID-19 are not known as they have not been explicitly tested in patients on haemodialysis. This study will involve having 3 blood tests to test for an antibody response following vaccination for COVID-19. The first will be 1 month after the first vaccination dose to look at the initial antibody response and the second and third will be 1 month and 6 months after the second vaccination dose.
