Validation of a Novel Sham Cervical Spinal Manipulation Procedure

Chronic Neck Pain

Primary hypotheses: Ho1: In chronic neck pain patients, randomly treated with either a "sham" or "real" cervical manipulation procedure, the probability of perceiving a Real Manipulation Procedure is equal to the probability of obtaining a Real Manipulation Procedure, AND, Ho2: Spinal tenderness will remain unchanged following the treatment procedure for both groups. Secondary (null) hypotheses: Neck pain, cervical ranges of motion and paraspinal EMG responses to pressure pain will remain unchanged in both groups; there will be no differences between groups with respect to 2a) neck joint cavitations and 2b) forces across the cervical spine measured during the manoeuvres.

Neck pain is a common complaint in the general population affecting 10-15% of the general population. females more than men. Known risk factors are: Trauma, Repetitive lifting of heavy objects at work, driving vibrating vehicles, frequents diving from boards and smoking. No study dealt with the possible role of unilateral loss of vision or hearing as a risk factor for the development of neck pain. Intuitively, one might suspect that unilateral blindness or deafness will lead to a tilted position of the head, in order to cover a better visual/audio field. This position, in time, might lead to the development of neck pain. Our hypothesis is that patients with unilateral blindness/deafness will suffer from neck pain more frequently than the general population.

It has been reported that excessive use of smartphones increases the stress on the cervical spine, and changes in cervical curvature and pain in the muscles around the neck are associated with the increase in stress. Studies have reported that smartphone users with addictions have neck pain (68%), upper back pain (62%), right shoulder pain (52%), left shoulder pain (46%), and right hand pain (46%). As a result of our study, the frequency of smartphone addiction and temporomandibular joint dysfunction and related factors in individuals with nonspecific neck pain were examined and the results we obtained will contribute to the literature.
