Fetal Tracheal Occlusion in Severe Diaphragmatic Hernia: a Randomized Trial

The neonatal mortality rate in cases with severe isolated congenital diaphragmatic hernia is higher than 90% due to severe pulmonary hypoplasia. Many studies have suggested that fetal tracheal occlusion may increase lung volumes and therefore reducing the risk for severe pulmonary hypoplasia and by consequence the risk for neonatal death. The main objective of the present study is to evaluate if fetal tracheal occlusion improves survival rate in those cases that are followed in our hospital, by conducing a randomized trial.

Rejuvenated, Washed Packed Red Blood Cells in Pediatric Cardiac Surgery

Blood transfusion is nearly always needed during open heart surgery in children less than 15 kg (35 pounds). The purpose of the red blood cells in the blood is to deliver oxygen to the organs and tissues of the body. Stored blood undergoes some changes that may make it less effective in achieving this goal. The purpose of this study is to see if restoring important energy molecules (ATP and 2,3,DPG) in stored red blood cells before they are transfused, with a rejuvenating solution (Rejuvesol), offers any advantages to children over standard blood transfusion. This is a Food and Drug Administration (FDA) approved process that is described by the American Association of Blood Banks for prolonging blood storage but not used for everyday transfusions. The investigators want to use this process to improve blood transfused to children undergoing heart surgery. Although Rejuvesol has been previously approved by the FDA, it is not routinely used to prepare standard blood transfusions to children undergoing surgery. Use of Rejuvesol in this study is considered investigational. This is a pilot study and data will be collected for future protocol development.

The goal of this study was to find prevalence and correlations of different weight abnormalities and risk of abdominal obesity in a random sample of Egyptian children suffering from cerebral palsy. This cross- sectional study hypothesize the prevalence of weight abnormalities and risk of abdominal obesity in addition to positive correlations between weight abnormalities and number of exercise hours and frequency of meals in a random sample of Egyptian children suffering from cerebral palsy RESEARCH QUESTION: What is the prevalence of weight abnormalities and and risk of abdominal obesity in Egyptian children with cerebral palsy? Does the weight abnormalities correlates with number of exercise hours or frequency of meals in children with cerebral palsy?

Enhancing Genomic Laboratory Reports to Enhance Communication and Empower Patients

Current lab reports are designed to communicate results from the laboratory to the provider; they are not designed to be accessible to patients. The investigators believe that a new type of genomic test report, tailored for patient- as well as provider-use, will enable patients to have access to information they can understand allowing them to be more involved in the management of their disorders, better navigate the health care system, and make more informed decisions about their health and health care in conjunction with their providers. This approach has the potential to improve outcomes from both the patient and provider perspectives. The investigators propose to study the research question, "Can a genomic laboratory report tailored for both providers and families of patients improve interpretation of complex results and facilitate recommended care by enhancing communication and shared decision making?"
