RCT on Effectiveness of Oral Health Preventive Programmes in School Children in Uganda

Dental caries represents a challenge for Oral Health Services in several African Countries, like Uganda. Few studies have been reported the burden of oral health, its prevention and non-operative treatment among school children in African countries. Aim: To assess the prevalence and risk factors of dental caries and the effectiveness of strategies to prevent and treat dental caries using a non-operative approach among children in Gulu Municipality, Uganda. Methods: The proposal RCT is structured in three phases: 1. A descriptive cross-sectional survey to collect oral data among 610 school children selected using a multistage cluster sampling; 2. A randomized unblinded two-arm trial to assess the difference in the treatment/prevention of dental caries using non-operative means among school children in the Ugandian Gulu municipality; and 3. A comparison of oral health conditions between a group of children who will be given dental cleaning tools and followed up over time versus a control group. The oral health status of the participants will be assessed according to WHO and ICDAS guidelines. Stata 14.0. will be used for analysis; descriptive statistics will be carried out to analyze continuous and categorical variables and chi-square test as well as independent tests for bivariate analysis and modified poisson regression. In addition, the factors associated with dental caries will be determined by linear regression models using a statistical significance level of 5% (α = 0.05). Discussion: This trial will be the first trial conducted in Uganda assessing a school-based caries prevention programme using the WHO and ICDAS standardized international guidelines. The findings obtained will increase knowledge on oral health in Uganda school children and the effectiveness of community-based caries prevention programme in this population.

Improved Oral Health and Dental Care in Children With Congenital Heart Disease

Background: Congenital heart disease (CHD) is one of the most common congenital anomalies in children. As the risk for endocarditis following bacteraemia is more common during daily oral hygiene routines, such as tooth brushing, than during dental treatment, the maintenance of optimal oral health in children with CHD may be considered of outmost importance. Children with CHD have a higher caries prevalence compared to healthy controls. Aim: The primary aim of the study is to explore if an educational intervention including information, counselling and support provided at early stage to families affected by major CHD, or CHD combined with a syndrome, can increase the awareness of the importance of maintaining of good oral health, and avoid the development of poor oral health including dental caries, leading to less dental anxiety and a higher oral heath related quality of life. The secondary aim is to elucidate factors associated with the development of poor oral health and/or orofacial dysfunction as well as family attitudes and needs of support. Hypothesis: Early information, counselling and support will lead to a) increased awareness of the importance to maintaining good oral health, b) prevent the development of poor oral health and dental caries, c) lead to less dental fear, and d) lead to higher oral heath related quality of life. Study design: Randomized educational intervention trial. Study population: Children born in Finland and under 12 months of age in 1.7.2017-31.12.2021 with, a) major CHD and potentially included in the criteria of endocarditis prophylaxis, or b) with a major CHD combined with a chromosomal syndrome, will be offered to participate in the study. Patients will be recruited until 200 patients are obtained at the Helsinki University Children's Hospital. A control group consisting of approximately 100 healthy children will also be recruited at birth. Main outcomes: Dental caries, dental anxiety, oral health related quality of life, and awareness of importance to maintain and behaviour to maintain good oral health. Significance: Better information to patients leads to better possibility to achieve good oral health. Better oral health leads to 1) less dental procedures and thereby less dental anxiety, 2) less dental procedures under general anaesthesia, which may potentially be a risk procedure for the child with CHD, and 3) better oral health related quality of life.

Epidemiological Profile of Children With Early Caries.

The objective of the study is to determine the different profiles of patients with early caries consulting at the Lille CHU and to analyse the characteristics associated with the severity of the pathology. The data will be collected using a simple clinical examination of the child and a survey of the parent using a questionnaire. The typology of patients with early caries will be analysed according to the predefined variables collected using the questionnaire via a main component analysis in order to study the links between these variables. At the same time, a hierarchical classification analysis will be carried out to study the existence of homogeneous sub-groups of individuals in regard to the early caries. In this study the objective is to establish in depth knowledge about adolescents' oral health characteristics, - practices and - needs. Further, we want to evaluate whether adolescents' caries status can be associated with existing socio-economic inequality, emotional health and lifestyle factors. The main question[s] it aims to answer are: What is the caries status (i.e. prevalence, increment at different timepoints and characteristics) among adolescents in Trøndelag? What is the caries prevalence in different age groups? Is there any difference in caries prevalence linked to specific age groups, gender or urban versus rural areas? Are approximal tooth surfaces especially prone to the development of new caries lesions? Is the caries increment from 12 to 20 years a continuous process or are there high-risk periods within this timespan? How is the prevalence of enamel caries? Is enamel caries more prevalent in younger age groups than in older age groups? What are the patterns of recall intervals and non-attendance in different age groups, among adolescents in Trøndelag? What are the attendance rates and prevalence of non-attendance at dental appointments in different age groups? What is the recall interval between regular follow-up dental visits? Are there any recall or attendance characteristics linked to age group, gender or urban versus rural areas? What factors (i.e. oral health behaviours, socioeconomic-, lifestyle- and parental factors) are associated with caries among Norwegian adolescents? How are the oral health behaviours (i.e. dietary- and oral hygiene habits, attendance to dental care) among adolescents, and are there any shared oral health behavioural challenges? Is the caries prevalence linked to life challenges (mental illness, alcohol/drug use and chronic diseases)? To what degree are oral health behaviours associated with caries among adolescents? Is there any association between media screen time and oral health behaviours, and cariesstatus? Is there any association between physical activity, sport participation and oral health behaviours, and caries status? What are the associations between socioeconomic factors (study program, birth- and migration background, and parental income, education and employment) and oral health behaviours, and caries status? For research question 1 and 2, the sample will be adolescents aged 12-20 years living in Trøndelag county in the time period 2008-2020, (n≈115000, Statistics Norway, SSB). Data will be extracted from dental health records in the public dental service (Den offentlige tannhelsetjenesten, Trøndelag Fylkeskommune). The public dental service annually reports caries prevalence for 12- and 18-year-olds, Statistics Norway, SSB (Figure 1). However, the knowledge is modest about the caries development within the six years from 12 to 18 years of age. Further, the public dental service offers subsidised dental care to 19- and 20-year-olds. Extracting data from dental health records in the public dental service will give us the opportunity to explore the caries increment at multiple timepoints, and attendance rates at dental appointments throughout the teenage period from 12 to 20 years of age. For research question 3, the sample will be adolescents (n≈8000) and their parents that participated in the fourth survey of The Trøndelag Health Study (HUNT4) in 2017-2019. Self-reported questionnaire data will be linked to data from dental health records in the public dental service.

Prevention of Recurrent Caries With Lactobacilli PRECAL

The aim of this study was to compare the effect of drops containing probiotic bacteria (Lactobacillus reuteri 17938, Lactobacillus reuteri PTA 5289, Biogaia AB, Sweden) on the incidence of dental caries with placebo drops in preschool children undergoing comprehensive restorative treatment under general anesthesia or conscious sedation. The project will be conducted at three specialist centers (Umeå University, Umeå, Sweden, Maxillo-facial unit, Halmstad Hospital, Sweden; Eastman Institute, Stockholm, Sweden) and employ a randomized placebo-controlled design with two parallel arms. The study will be performed double-blind and neither the parents nor the clinicians will be aware of the content of the drops. The follow-up period is one year.
