Tofacitinib for Immune Skin Conditions in Down Syndrome
Down SyndromeAlopecia Areata4 morePeople with Down syndrome (DS) display widespread immune dysregulation, including several immune skin conditions. This study hypothesizes that pharmacological inhibition of the increased interferon (IFN) signaling seen in DS is safe and could improve associated skin conditions. The study evaluates the safety and efficacy treatment with Tofacitinib, an FDA-approved drug known to block IFN signaling, in adolescents and adults with DS and an autoimmune and/or autoinflammatory skin condition. Investigators will also measure the impact of interferon inhibition on a variety of molecular markers, as well as the cognitive abilities and quality of life of participants.

Repurposing Dupilumab for Management of Pruritic Genetic Inflammatory Skin Disorders
Skin DiseasesSevere itch is a common symptom of many genetic skin disorders and leads to a negative impact on patient quality of life. The investigators hypothesize that: a) intervention with dupilumab will improve itch in patients with pruritic genetic inflammatory skin disorders, even those not recognized to be Th2-driven; and b) the administration of dupilumab will be well-tolerated, regardless of underlying genetic skin disorder. The total clinical study duration will be 26 months (104 Weeks). The treatment period will include a 16-week open-label phase and a 20-month long-term extension phase for those who qualify and wish to continue.
Mindful Parenting - A Single Case Experimental Design Study
ParentingAlopecia5 moreSome parents of children living with a visible difference can experience heightened stress due to the associated challenges of this. Parent's views of the child's visible difference and their responses to the child are important. Mindful parenting approaches have been found to reduce stress or distress for parents of children with disabilities, physical health problems and skin conditions. This study will be completed with a small number of participants (around six to 12). Participants will be parents or carers of a child living with a visible difference aged four to 16 years, who are experiencing stress. Parents or carers will complete an online mindful parenting intervention (called Two Hearts) including video content, audio files and a workbook, over six weeks. Over the twelve-week study period participants will also provide the following information which will be compared over time: Complete four group support sessions during the intervention via videoconferencing Complete questionnaires at four timepoints Provide information about their use of the intervention materials and home practice weekly Answer two questions daily via text message about parenting stress levels We hope to learn about the initial effects of the mindful parenting programme for parents or carers of children living with a visible difference. We also hope to learn whether parents or carers find completing an online programme possible and practical. Finally, we hope to learn what parents' or carers' views are of the online programme and whether this type of intervention in online format would be helpful for other parents or carers.

EczemaDermatitis5 moreBackground: - Atopic dermatitis, or eczema, is a chronic skin disorder. Patients sometimes have infections with S. aureus bacteria. Researchers want to study how eczema treatments affect the number and the type of bacteria on the skin. Objectives: - To study the effect of eczema treatments on skin bacteria. Eligibility: Individuals between 2 and 25 years of age who have moderate to severe atopic dermatitis. Healthy volunteers between 18 and 40 years of age with no history of eczema. Design: Participants will be screened with a physical exam and medical history. Research samples will be collected. Skin biopsies may also be performed. All participants will be assigned to one of several study groups. This study will last for up to 1 year. Healthy volunteers must not have taken antibiotics in the year before the start of the study. All participants will have regular study visits during their 1-year participation. More research samples will be collected at these visits.
