Balance, Plantar Pressure Distribution and Gait in Pes Planus

Introduction:The results of studies evaluating balance, plantar pressure analysis, and gait in adults with pes planus have differed in the literature. The aim of this study is to evaluate balance, plantar pressure, and gait in adults with pes planus and compare them with adults without pes planus. Materials and Methods: The study will be conducted at the Ministry of Youth and Sports, Athletic Training and Research Center. A total of 118 individuals, 59 individuals with pes planus and 59 individuals without pes planus, will be included in the study according to the Navicular Drop Test and Foot Posture Index. In the study, the descriptive characteristics of the individuals will be recorded. Static and dynamic balance measurements, plantar pressure analysis and gait analysis of individuals will be performed, and the two groups will be compared. Results: The data will be analyzed using the statistical program for social sciences (SPSS) version 21.0 (IBM SPSS Statistics for Windows, Armonk, NY: IBM Corp.). The data will be expressed as mean standard deviation (X±SD) and number (n%). The homogeneity of the groups will be evaluated with the Levene Test. Balance, plantar pressure, and walking values between the groups will be compared using the "Mann Whitney-U" Test. All the statistical analyses will be set a priori at an alpha level of p<0.05. Discussion: The results will be discussed in the light of the recent literature. In the literature, there are conflicting results about the balance and gait of adults with pes planus. The study can contribute to the literature in this respect.

Outcomes of the Evans Calcaneal Lengthening Based on Bone Grafting Material

The Evans calcaneal lengthening is a common surgical correction used for children with spastic cerebral palsy (CP) and painful flat feet. This procedure involves the insertion of a trapezoidal bone graft into the calcaneus bone. Both autograft and allografting materials have been used for this procedure, but both types of grafting materials have associated limitations. Autologous grafting materials, which are harvested from the iliac crest at the time of surgery, are associated with high rates of prolonged donor site pain. There is limited bone available from the iliac crest. For this reason autologous bone grafts are not commonly used for the Evan's procedure. Cadaver allografting materials are variable in their mechanical and geometric properties. Recently, xenografting materials have been developed that have consistent mechanical properties and are available in a larger size that is formed to fit the individual patient intraoperatively. The purpose of this study is to evaluate the long term outcomes of the procedure with the different graft options. Another purpose is to compare the difference in surgical correction obtained with the cadaver allograft and the xenografting materials. This study will also observe and evaluate a wide range of outcome variables that encompass multiple levels of patient care, from clinical observation to parent and patient satisfaction, and compare these with the observed changes in the radiographic and plantar pressure parameters of the foot. This data will be evaluated pre to post surgery and for two years of follow-up time. The aim is to uncover the characteristics of collapse observed with the two different grafting materials.

Pes Planus

Pes planus is a deformity called "flatfoot" and is defined as the valgus of the hindfoot while loading the foot, the disappearance of the medial longitudinal arch in the midfoot, and the supination of the forefoot relative to the hindfoot. The aim of this study is to compare the subtalar angle, q angle, trunk muscle endurance, and performance of individuals with pes planus and healthy controls. Volunteer students between the ages of 18-35 will be included among the students studying at the Health Sciences University Gülhane Faculty of Physiotherapy and Rehabilitation. The feet of individuals who want to be included in the study will be examined by the researchers with the navicular drop test and the presence of pes planus will be determined according to this test. Subtalar Angle to determine the degree of rotation (Varus/Valgus) in the hindfoot, Q Angle for knee biomechanics, Navicular Drop Test to determine flat feet, Mcgill trunk Flexion, Extension, and Lateral Bridge Test to evaluate trunk endurance, Xbox to evaluate lower and upper extremity performance 360Kinect™ game console will be used. In this study, statistical analyzes will be made with SPSS 21 package program (IBM Corp., Armonk, NY, USA). Wilcoxon test or Mann Whitney U will be used after determining the conformity to the normal distribution in the comparison of the values before and after the exercises, individuals with and without flat feet. Results will be evaluated at the significance level of p<0.05. Cohen's d values will be calculated to determine the study effect size.

Effect of Medial Wedge on Static Balance in Pronated Feet

The use of heel wedges is often recommended as a clinical routine in individuals with foot pronation. However, there is a lack of information for examining the immediate effect of supports used to restore foot biomechanics on balance. The aim of our study is to examine the immediate effect of calcaneal support in the frontal plane on static balance in individuals with increased pronated foot. In this study, the fore-hind foot load distribution in static bipedal stance will be examined in healthy young adults. Then, static balance measurements will be made on one leg with and without support (medial heel wedge). For the evaluation of static balance on one leg, x-y mean, ellipse surface, A-P index measurements and romberg test will be used. In addition, the pain of individuals will be questioned and their foot postures will be evaluated. Healthy young adult individuals with a subtalar angle of 5 degrees and above in the weighted position will be included in the evaluations. Evaluations of the participants are planned to take approximately 15 minutes.
