Kidney DiseasesThis protocol seeks to define aspects of intestinal and hepatic uptake and metabolism of several amino acids. The major hypothesis to be tested is that the splanchic bed (intestine and liver) conserves essential amino acids and metabolites while synthesizing and metabolizing nonessential amino acids. The aims of the study include defining the relative roles of enteral and hepatic extraction/metabolism in the disposition of glutamate, alanine, methionine and other amino acids. Emphasis will also be placed on studies of the transamination of alpha-ketoisocaproic acid (KIC) to leucine since this reaction is of potential importance in the design of nutritional regimens for patients with renal disease.

OBJETIVES Primary objective Estimate the contribution of renal dysfunction to endothelial dysfunction in two cohorts of patients, living kidney donors and their transplant recipients. Secondary objectives To evaluate in both cohorts of patients before and after nephrectomy/transplantation the evolution of the following parameters: Renal function (iohexolGFR, proteinuria/microalbuminuria). Blood pressure (24 h ambulatory blood pressure measurement) Surrogate variables of subclinical atherosclerosis (carotid ultrasound, ankle-brachial index, pulse wave velocity). Identification of Corticomedullary Differentiation Along Different Stage of Pregnancy
NephropathyCorticomedullary differentiation (CMD) abnormality (absent/reversed) is a parameter that can indicate possible nephropathy. The evolution of CMD across gestation had not been well established. Devrendt et al demonstrated the presence of CMD in all fetuses older then 20 weeks. In our study we would like to determine the exact timing of it's sonographic visualisation. This is a prospective study recruiting women between 14-24 week gestation during routine ultrasound screening. A midsagittal image of each kidney will be taken by a single sonographer and examined for the presence of the CMD by two other sonographers blinded to gestational age.
