Home-based Preoperative Exercise Training for Lung Cancer Patients Undergoing Surgery

Lung Neoplasm

Lung cancer accounts for over 11% of global cancer incidence and is the leading cause of cancer death, with numbers in 2020 reaching 1.8 million deaths worldwide. For early-stage lung cancer patients, surgical resection is the recommended treatment and the intervention associated with a better prognosis. However, in consequence of surgery there is a substantial deterioration in health-related quality of life across most dimensions, especially in the first month, with 100% of lung cancer patients concerned about the limitations in their physical function and 96% about the levels of fatigue and pain after lobectomy. Additionally, some patients developed postoperative pulmonary complications, which are associated with increased length of hospital stay and higher risk of mortality. In this context, and considering that the number of lung cancer cases with an indication for surgery will increase by 60% from 2018 to 2040, to find feasible and effective interventions that could optimize postoperative recovery is of major clinical relevance. The primary purpose of this study will be to evaluate the efficacy of home-based preoperative exercise training to improve health-related quality of life after lung cancer surgery. The secondary purpose of this study will be to evaluate the efficacy of the home-based exercise program to improve physical performance and to reduce postoperative complications /length of hospital stay. Participants will be randomly allocated to a preoperative exercise intervention, that will consist of combined aerobic and resistance exercise, or to a control group that will receive usual care (i.e., no exercise training). Based on the strong evidence indicating a therapeutic effect of exercise training on fatigue and physical function, domains of HRQOL especially affected after lung cancer surgery, the investigators hypothesized that the home-based exercise program will be effective to improve these domains before surgery and attenuate its deterioration after surgery, optimizing the recovery in postoperative HRQOL.

The Use of Molecular Radiogenomics in Non-small Cell Lung Cancer

Lung Cancer

Lung cancer is currently the leading cause of cancer-related mortality worldwide, and the dominant histopathology is non-small cell lung cancer (NSCLC). Although many new targeted and immunomodulation therapies have emerged, not all patients are responsive to novel therapeutics. A more reliable and accurate risk stratification model to predict the treatment response and survival outcomes are still lacking. The 18F-fluorodeoxyglucose (18F-FDG) positron emission tomography (PET) derived radiomics can be used to interrogate tumor biologies such as glycolytic activity and heterogeneity. It can, therefore, be used to predict treatment response and survival outcomes. Cancer genomics derived from gene sequencing can evaluate cancer's genetic alterations. It can be used to feature the genotype of the tumor. However, both tools have drawbacks; combining these two modalities may enable a more robust predictive model for more precise clinical decisions. During the investigator's former study project, the investigators published four Science Citation Index journal papers using the investigators' research results, which found that 18F-FDG PET radiomics can independently predict regional lymph node metastasis in NSCLC and cancer survival by stage. The preliminary findings of the investigator's former research project also disclosed an association between 18F-FDG PET-derived molecular radiomics with genomic heterogeneity and mutation of specific glucose metabolic genes. This time, the investigators plan to include deep radiomics in addition to traditional handcrafted radiomics. The investigators aim to investigate the radiogenomic patterns in different driver gene mutation statuses and clinical scenarios. Finally, the investigators seek to use radiogenomics as a prognostic stratification tool in patients with NSCLC.

PLAsma Genomic Testing in Patients With Advanced Non-Small Cell Lung Cancer: The PLAN Study

Non-small Cell Lung Cancer

This is a multi-centre, prospective, translational study investigating the use of plasma genotyping for initial genomic testing in newly diagnosed advanced/locally advanced non-squamous NSCLC. In this study, patients will have a plasma genotyping assay completed following confirmation of suspected diagnosis of non-squamous NSCLC at institutional Rapid Access Lung Cancer Clinics (RALCC), alongside standard tissue-based biopsy and genotyping.

Prognostic Value of Lung Cancer MicroAnatomy in 3D

Lung CancerAdenocarcinoma of Lung4 more

Micro-computed tomography (micro-CT) is a novel biomedical non-destructive, slide-free digital imaging modality, which enables the rapid acquisition of accurate high-resolution, volumetric images of intact surgical tissue specimens. This imaging modality provides microscopic level of detail of intact tissues in three-dimensions without requiring any specimen preparation. Its non-destructive nature and the ongoing enhancement of imaging resolution and contrast renders micro-CT imaging particularly well suited for microanatomic studies in basic research across a wide range of interventional medical disciplines, including oncology. Our proposal concerns a multidisciplinary basic research effort which aims to facilitate the effective identification of different -and maybe challenging to differentiate- lung cancer patterns based on 3D X-ray histology. As an alternative for the use of hematoxylin & eosin (H&E) slides, optimized micro-CT scanning of soft tissues emerges as a promising tool to enable non-invasive 3D X-ray histology of formalin-fixed and paraffin-embedded (FFPE) lung cancer specimens. The objective of our proposal is to offer novel insights into the complex architecture of each lung cancer subtype after imaging FFPE surgical specimens, resected from lung cancer surgeries. The investigators aim to generate 3D datasets of FFPE lung cancer tissues which will be combined with the corresponding conventional 2D histology slides. Our study will be also adequately empowered to identify particular differences in morphometric measurements according to each particular lung cancer growth pattern. Finally, this proposal aims to delineate the different 3D microanatomy and morphology of some patterns that are challenging to interpret and differentiate through traditional 2D histological evaluation, such as papillary and lepidic adenocarcinoma growth patterns. Classification of the histological subtypes based on 2D histology sections can be ambiguous, as shown by suboptimal inter-observer consensus when determining predominant histological subtypes in FFPE lung adenocarcinoma tissue specimens. Hence, micro-CT-based 3D imaging of the lung specimens could aid classification of histological subtypes by providing more comprehensive sampling of the entire tissue block and yielding detail relevant for subtype classification that might not be visible in 2D sections alone.

Lung Cancer Cryo-Activation as a Novel Approach to Augment Immunotherapy Efficacy (CRYOVATE)

Non-small Cell Lung Cancer MetastaticCryotherapy Effect

Cryo-activation involves the insertion of a cryoprobe in the tumor bed with subsequent cell necrosis and tumor antigens release. Such technique has the potential to induce immune-specific reactions influencing cancer cells outside of the ablated region. The addition of cryo-activation to immune-checkpoint blockers (ICB) in the advanced NSCLC setting could represent a synergistic therapeutic avenue in order to potentiate treatment responses

Narrow Band Imaging (NBI) Under Electronic Bronchoscope in Lung Cancer

Lung CancerMalignant Airway Obstruction

Narrow-Band Imaging (NBI) is useful to better demarcate the superficial extent of central type of lung cancer, but its sensitivity and specificity in clinical practice were little studied. This study aimed to investigate the diagnostic effects of NBI in suspected patients with central lung cancer and its application in staging diagnosis of central lung cancer.
