Cognitive Behavioural Therapy to Optimize Post-Operative Recovery Trial
PainPostoperative8 morePsychological factors such as stress, distress, anxiety, depression, and poor coping strategies may be associated with ongoing pain following injuries such as fractures. To study this relationship, patients will undergo cognitive behavioural therapy (CBT) which is designed to modify such thoughts with the goal of reducing ongoing pain and improving quality of life. The goal of this study is to determine if CBT, versus usual care, reduces the prevalence of moderate to severe persistent post-surgical pain (PPSP) over 12-months post-fracture in patients with an open or closed fracture of the appendicular skeleton, treated with internal fixation.
Immune Mechanisms of Vitamin D to Reduce Chronic Pain After Burn
Burn InjuryChronic PainThe goal of this pilot clinical trial is to learn whether vitamin D is able to prevent chronic pain following burn injury and to determine what biological mechanisms are engaged by Vitamin D following burn injury. The main question[s] it aims to answer are: Is the clinical trial protocol feasible? Is Vitamin D administration following burn injury safe? How does vitamin D cause changes in the immune system in the aftermath of burn injury? Following informed consent, participants will be asked to: Take 6 capsules by mouth one time following burn injury (Vitamin D or Placebo) Provide a blood sample at baseline and 6 weeks following injury Fill out surveys daily while in the hospital, weekly through 6 weeks, and at 3 months and 6 months. Researchers will compare Vitamin D and placebo groups to see if there are differences in adverse effects (side effects), chronic pain, and profiles of immune cells from collected blood samples.
Transitioning to a Valve -Gated Intrathecal Drug Delivery System
PainPain5 moreThis study is designed to explore if the valve-gated pump requires less drug to manage subject pain than the prior standard peristaltic pump in the same subject. The newly implanted valve-gated pump will be programmed to deliver a minimum dose reduction of 20% of the same medication that was delivered in the peristaltic pump prior to explant. The drug therapy will be evaluated and pain scores will be evaluated over time (3 refill cycles prospectively for the valve-gated pump and 6 months retrospectively for the peristaltic pump).

This investigation will examine the impacts of green light exposure on subject-reported pain severity and symptoms. Knowing whether this intervention can improve pain and quality of life in this population may offer valuable guidance to clinicians who treat hEDS patients and to hEDS patients themselves.

Hematopoietic and Lymphoid Cell NeoplasmMalignant Solid NeoplasmThis phase I trial evaluates the side effects of psilocybin and how well it works under supportive care conditions in cancer survivors living with demoralization and chronic pain. Cancer patients often experience demoralization, which is characterized by feelings of hopelessness, loss of meaning, and existential distress. Psilocybin psychotherapy, together with multidisciplinary palliative and supportive care, may help treat the anxiety, depression, and chronic pain felt by cancer survivors - defined here as cancer patients from time of diagnosis through the end-of-life.

Chronic PainLow Back PainChronic pain causes immense suffering and reductions in quality of life as well as enormous socioeconomic costs. Very many chronic pain patients fall into the category of unspecific pain, i.e. pain without clear medical explanation, with lacking effective treatments. It is assumed that a negative hedonic shift, characterized by excessive emotional-motivational processing and neg-ative affect, contributes causally to the development and maintenance of chronic pain. The mechanisms leading to such a shift are largely unclear; however, learning mechanisms appear likely candidates, possibly causing decreased connectivity in the fronto-striatal brain circuits. The project's over-all aim is to characterize mechanisms of emotional-motivational pain pro-cessing. The specific objectives are to illustrate that emotional-motivational pain components are heightened in chronic pain and that they can be de-creased by counterconditioning as an important and pervasive mechanisms in everyday life. Furthermore, its neural correlates in fronto-striatal networks underlying the conditioning effects will be characterized.
