Post-operative Pain After Shoulder Surgery

PainShoulder

A prospective observational study conducted at the American University of Beirut Medical center that targets patients undergoing shoulder surgeries. It is conducted to to prospectively collect patient, surgery and anesthesia-related variables and perform correlation studies with postoperative pain severity, patients' satisfaction and analgesic consumption following shoulder surgery. In addition, identifying predictors for postoperative pain will contribute to adjust pain management protocols as per surgical and patients' factors.

Validation of a Score for Shoulder Function Evaluation Based on Movement Analysis

Questionnaires are frequently used to evaluate shoulder function in various diseases or after surgery. However, measurement of shoulder function is presently a controversial issue. Shoulder movement analysis based on embedded sensors could be a promising alternative to questionnaires. Some studies already demonstrated the relevance of this approach. It has also been demonstrated that a simple testing procedure including only two arm movements produces comparable results to more complicated testing procedures. However, more studies are needed to extensively establish if this simplified testing procedure provides a trustworthy evaluation of patient shoulder function and its evolution. This study is a preliminary study which aims to develop a precise testing procedure which will be used in a future study aiming to evaluate measurement properties of a simple shoulder function test based on movement sensors.

Reliability and Validity of Goniometric iPhone Applications

The purpose of this study is to determine the test-retest and inter-rater reliability and concurrent validity of two iPhone goniometric applications in the measurement of active shoulder external rotation as compared to a standard manual goniometer. The scientific hypotheses for this study are as follows: There will be significant Pearson correlations (>.8) to demonstrate test-retest reliability for each of the tools between the 1st and 2nd round of testing as assessed by novice testers. There will be significant intra-class correlation coefficients (>.8) to demonstrate inter-rater reliability between the novice tester and the experienced tester for each of the three tools. There will be significant Pearson correlations (>.8) to demonstrate concurrent validity between the standard goniometer and the photo capture application, the standard goniometer and the accelerometer-based application and the photo capture application with the accelerometer-based app. as assessed by novice testers.

Lavage and Suction of the Right Upper Quadrant to Reduce Post Laparoscopic Shoulder Pain

The use of laparoscopy in gynecologic surgery has been well established to decrease morbidity, blood loss, hospital stay, and post-operative pain when compared to traditional open abdominal surgery. However, the laparoscopic technique is associated with post-operative shoulder pain. We hypothesize that a combination of intraperitoneal saline lavage and active suction removal of carbon dioxide gas from the right upper quadrant of the abdomen will decrease incidence of post-laparoscopic shoulder pain when compared to passive exsufflation of carbon dioxide gas.

A Study of the Incidence of Post-Thoracotomy Shoulder Pain

Post-thoracotomy Shoulder Pain

Patients undergoing chest surgery (thoracotomy) often develop shoulder pain after their operation despite epidural pain control of the incision. We plan to study the incidence of this phenomenon in our institution to determine whether there are patterns that may predict those persons at highest risk of developing this type of pain. Further, we hope to determine whether newer surgical techniques have an impact on the development of this type of pain.

The Turkish Version of the WUSPI

Wheelchair users frequently experience shoulder pain. It has been determined that in our country, the subjective assessment methods that will help to determine and monitor shoulder pain severity of wheelchair users are limited. The purpose of this study is to adapt the original English version of the Wheelchair Users Shoulder Pain Index into Turkish and to assess its validity and reliability. Firstly, the original English version of WUSPI was translated into Turkish and culturally adapted, then applied to 100 wheelchair users to investigate the validity of the scale. The reliability of the scale was assessed by internal consistency and test-retest analysis and found a high internal consistency. Validation of WUSPI-Tr was assessed by the concurrent validity method. To establish concurrent validity, WUSPI scores were compared to SPADI and ASES scores by Spearman correlation analysis. There was a positively excellent correlation with SPADI and, negatively excellent correlation with ASES. As a result, it is determined tath the Turkish version of WUSPI-Tr is a valid and reliable tool.

Shoulder pain is a well known problem after thoracical surgery. The pathogenesis of the pain is uncertain, but mainly a mix of referred pain and mechanical strain to the shoulder. It is believed to last a few days. Persisting shoulder pain after thoracical surgery is a problem 1 year after surgery, but it is unknown if it is related to acute postoperative shoulder pain. The purpose of the study is to systematically describe postoperative shoulder pain after lobectomy and examine the time progress up to 1 year after surgery.

Reducing Shoulder Tip Pain Following Laparoscopic Surgery

Laparoscopic surgery is becoming a major procedure, owing to smaller incisions, shorter hospitalizations, and less post-operative pain as compared with traditional laparotomies. However, there is marked interindividual variability of post-operative shoulder-tip pain following laparoscopic surgery. The incidence of shoulder pain varies from 35% to 80% and ranges from mild to severe. In some cases, it has been reported to last more than 72 hours after surgery. The hypothesis of post-operative shoulder-tip pain is that carbon dioxide induced phrenic nerve irritation causes referred pain to C4. Therefore, the investigators should try to reduce carbon dioxide retention in the pelvic cavity.
