Does Obesity Increase the Risk of Conversion and Short Term Complications in Laparoscopic Rectal...

Obesity is on the rise in the Western population and BMI has been shown to be associated with an increased risk of per- and postoperative complications. The investigators intend to study a population of more than 300 patients undergoing laparoscopic surgery for rectal cancer. The investigators main outcome measure will be the conversion rate, and the investigators also intend to study other indications of short term complications, such as peroperative bleeding, infection, re-operation and mortality. The investigators hypothesise that increased BMI does not increase the risk of conversion.

Compromised peripheral tissue oxygenation during surgery may lead to worse patient outcome, mainly due to post-operative infections or heart failure. Insufficient stroke volume and/or cardiac output due to hypovolemia or cardiac defects play a central role in causing poor peripheral tissue oxygenation. In order to assess stroke volume, there are numerous invasive and non-invasive methods available. Up to the present date it is unknown, if these methods may by used interchangeably in patients with severe cardiac defects like aortic stenosis.

Limbus Corneae

The limbus located between the cornea and the conjunctiva tissue, is important for not only providing a barrier frontier to prevent conjunctival tissue invasion into the cornea, containing nerves passing to the cornea, having blood and lymph vasculature for oxygen and nutrient delivery, but also the niche environment of limbal stem cells. So far, in vivo image systems are not able to visualize or identify the limbal stem cells directly. One alternative practical is to visualize the histological morphology of palisades of Vogt (POV), and to speculate the possible status of the stem cells accordingly. Slit-lamp biomicroscope can be used routinely for clinical examination of the limbal morphology. However, this technology does not allow for high resolution imaging of structural details and only up to 20% of patients can be identified. In vivo confocal microscopy has been used to visualize the POV and can provide cellular level resolution images, but the technique is limited by high magnification that restricts the area of the scan, and requires contact with the eye. Besides, both slit lamp biomicroscopy and in vivo confocal microscopy have the limitation of not being able to give an overall view of the dimension and structure of the whole palisades region. Anterior segment optical coherence tomography (AS-OCT) is a noninvasive, rapid and reproducible technique to evaluate the anterior segment and can also provide in vivo spatial information. The purpose of the study is to assess the role of aging and regions on the limbus.

Limited data exist on early diagnosis and prevention of these complications. The investigators undertook this prospective study to determine the incidence, predictor factors, specific pathogenesis, management and survival specially for patients with thrombotic and bleeding complication to better improve outcomes.

Transvaginal Treatment of Symptomatic Cystocele Grade II-III

Cystocele

This observational study will evaluate treatment of symptomatic cystocele grade II-III with Cousin Biomesh and Allium Medical Endofast Reliant. Data about 100 patients will be collected. Follow-up till 3 years after the procedure. Primary endpoint is absence of recurrence.
