A Study of the Relationship Between Disulfiram and Cocaine Self-administration.

Cocaine Addiction

The problem of cocaine dependence remains a major medical, social, and legal concern. Several studies have suggested that disulfiram may be beneficial for the treatment of cocaine dependence. A common assumption has been that disulfiram treatment, by increasing DA availability, enhances the aversive aspects of stimulants. This study aims to measure plasma activity in those with the C/C DBH genotype, which is associated with higher DBH activity subsequently making the disulfiram treatment more effective, as well as determine the effects of treatment with disulfiram on cocaine self-administration using a human laboratory model of cocaine self-administration.

Integration of Buprenorphine Into HIV Clinical Settings - Primary Care Model (PCM)

The purpose of this study is to assess the feasibility, cost and effectiveness of interventions designed to integrate buprenorphine treatment for opioid dependence into HIV primary care in ten HIV care centers in the U.S. In the site led by Dr. Altice, we compare two models of providing HIV care and buprenorphine treatment. Assignments are based on participants' city of residence. In the onsite (integrated care) model, participants receive buprenorphine, substance abuse counseling and HIV care at one location: the Waterbury Hospital Infectious Disease Clinic. In the off-site model (non-integrated care) buprenorphine induction, substance abuse counseling, and HIV care will be provided at separate locations: the Community Health Care Van (CHCV), the Yale AIDS Program, and patients' own HIV providers, respectively. Data is collected from interviews with participants, reviews of medical records, and surveys and interviews with clinicians.

Assessment of GVG for the Treatment of Methamphetamine Dependence

Methamphetamine Addiction

The purpose of this study is to find out if GVG can reduce drug use and determine safety and effects of GVG when used together with methamphetamine. This study involves staying in the hospital for 21 days. Participants will receive either placebo or GVG, and a limited amount if methamphetamine will be injected on some study days. This study will enroll people that use methamphetamine. Participants will be compensated.

Although the use of opioid medications has gained increasing acceptance as a treatment option for chronic pain, there is little empirical research concerning long-term outcomes with this patient population. Several important questions concerning the use of opioids have yet to be examined in controlled studies. These include: determination of the most effective dosing regime, specification of the risk factors for medication abuse, and identification of patients most likely to achieve long-term benefits. Despite the lack of empirical data, the clinical literature to date yields two general approaches to the use of opioid treatment with chronic, nonmalignant pain. The first approach dictates that due to risk of addiction, tolerance, and insensitivity to large doses, opioid medications should be carefully controlled, with minimal and stable dosing as the eventual goal of treatment. The second approach, fostered by the beneficial use of opioids in the treatment of cancer pain, maintains that risk of addiction and abuse has been exaggerated. Proponents of this approach suggest that under-medication is a significant cause of treatment failure and non-compliance in chronic pain patients, and that prescription guidelines should emphasize patients� reports of adequate relief, regardless of dosage.

This study aims to determine the effectiveness of the group psycho-education program which is prepared as "cognitive behavioural-based", on college students' smartphone dependency levels. The research will be carried out on students studying at a private foundation University in 2019-2020 academic year. In this study, pre-test-final Test control group and follow-up measured experimental pattern will be used to determine the effect of Psycho-education program on smartphone dependency levels of college students. A Personal Information Form and Smartphone Dependency scale will be applied to students who agree to participate in the study. By analysing the data results obtained from the data collection tools and passing the determined limit value, 94 people will be randomly selected, including 47 experiments and 47 control groups, from those who scored high on the smartphone dependency scale through the SPSS program using the power analysis method. Students will be informed about cognitive behavioural based psychoeducation and will be asked if they wish to participate in the training. Psychoeducation Practice 1 day per week, 60-90 min. sessions will take 8 weeks. At the end of the training, the final test will be applied to the experimental and control group. 3rd and 6th month follow-up measurements are planned to be carried out intermittently at the end of the month. In the literature, it is observed that the number of intervention studies is very small, and no clear conclusions have been reached. In addition, the treatment of smartphone addiction in Turkey with cognitive-behavioural approach-based group psychoeducation program has not been studied before. In our country, where smartphone addiction is increasing every day, the importance of intervention efforts in this area is clear. It is anticipated that the intervention study on addiction reduction will be supportive for researchers and will be beneficial for people who need support for smartphone addiction. In this respect, it is thought that it will be a guiding work for future clinical and academic studies.

A Electronic System to Improve Recovery Outcomes in Patients With Drug Use Disorder

Background: Relapse is very common in patients with illicit drug use disorder and contributes to a series of bad consequences that substantially impact patients' physical and social functions. Due to the limited existence of effective addiction treatment, the majority of patients with drug use disorder could not access help when needing. Mobile health (mHealth) offers a potential solution to improving recovery outcome for patients in community. Objective: This paper is a protocol for a randomized controlled trial (RCT) of a smart phone application called community-based addiction rehabilitation electronic system (CAREs) . This interactive system consists of an APP for clients and a webpage for service providers, with the aim of teaching clients craving and emergency coping skills and helping service providers to improve work efficiency and effectiveness in community. Methods: A randomized controlled trial(RCT) will be conducted. Sixty drug illicit users who are newly ordered to undergo community rehabilitation will be recruited from the community in Shanghai. Participants will be 1:1 randomly assigned to receive integrated community rehabilitation by using CAREs or only receiving routine community rehabilitation for 6 months. Corresponding anti-drug social workers will provide service and monitor participants' drug use behavior in accordance with the routine work-flow. Outcomes will be assessed at baseline and in the 6th month. The primary study outcome is the performance on illicit drug urine test which will be carried on regularly twice per week during the study period. Secondary study outcomes include days that participants interact with anti-drug social workers, and the decrease rate of addiction-related issues severity index.

The current research project constitutes a highly novel approach to understanding the pathophysiology of addiction through its combination of neuroimaging with state-of-the-art information concerning the real-time expression of risk factors.
