Understanding Control and Mechanisms of Shoulder Instability in FSHD

The aim of this study is to identify factors for shoulder instability in people with Facioscapulohumeral dystrophy (FSHD). FSHD is a non-life limiting condition with symptoms presenting in the second decade of life (Evangelista et al., 2016). Between 2500 to 3000 people are diagnosed with FSHD in the UK and it is the third most common dystrophy. The overall prevalence is 1: 20,000 and on average 52 people are newly diagnosed with FSHD each year (Emery, 1991; Padberg et al., 1995; UK, 2020) As the disease progresses, patients lose the ability to adequately control muscles around the shoulder girdle, possibly contributing to the development of shoulder instability i.e. partial or complete dislocation of the shoulder joint (Bergsma, Cup, Geurts, & De Groot, 2015; Bergsma, Cup, Janssen, Geurts, & de Groot, 2017; Mul et al., 2016). Loss of control around the shoulder is also thought to contribute to pain and a reduced capacity to perform tasks above shoulder height. Additionally, the development of fatigue and chronic pain further limit patient's abilities and engagement with rehabilitation. If we better understand the mechanisms associated with instability, we can better target physiotherapy interventions to improve rehabilitation. If we identify specific patterns of activity associated with instability, these could be addressed through personalised and improved exercise prescription and rehabilitation. Additionally, we may identify causes of instability for which physiotherapy or exercise programmes may not be appropriate, therefore ensuring patients are referred to the correct service in a timely manner, improving patient outcomes and allocating resources more appropriately.

Shoulder Kinematics and Muscle Activation in Sport Climbers With Shoulder Pain

Sport climbing has recently become a highly popular sport and was included in the Summer Olympic Games 2021. Shoulder injury accounts for 15 to 20 percent of all sport climbing injuries, and it is the third most common problems, only surpassed by wrist and hand injuries. Sport climbing involves both open and closed chain movements of upper limbs. These prolonged and repetitive movements on vertical or overhanging terrain may lead to overuse and fatigue, which result in injuries. Sport climbing consists of multiple movement elements. Pull up with small holds is a basic movement in sport climbing, and activates many shoulder muscles, including trapezius, serratus anterior, latissimus dorsi. Studies related to sport climbing have focused on observing difference between sport climbers and non-sport climbers. Altered posture, increased scapulohumeral rhythm during arm elevation, and decreased shoulder angle during one-arm hanging have been observed in sport climbers. However, little studies have investigated whether climbers with shoulder injuries demonstrate different shoulder kinematics and muscle activation during tasks related to sport climbing. Therefore, the purpose of this study is to investigate the shoulder kinematics and muscle activation during pull-up with small holds in sport climbers with and without shoulder pain. Methods. Thirty sport climbers with shoulder pain and 30 healthy control subjects matched with gender, age, dominant hand and experience will be recruited in this study. All subjects will undertake physical examinations such as shoulder range of motion (ROM). Shoulder kinematics and muscle activation will be collected during arm elevation in the scapular plane and pull-up with gym-bar and 20mm depth small climbing holds. An electromagnetic tracking system will be used to measure bilateral scapular and humerothoracic kinematics. Surface electromyography (EMG) will be used to record muscle activation of bilateral upper trapezius, lower trapezius, serratus anterior and latissimus dorsi. Demographic data and physical examination will be analyzed by independent t test or Chi-square test. Two-way ANOVA will be used to test differences between groups in kinematics and EMG data at different phases of tasks. If significant interaction effect is found, Post-hoc pairwise comparison with Bonferroni correction will be used. The significant level is set at 0.05.

This study investigates the agreement between extended scope physiotherapists (ESP) and orthopaedic surgeons (OS) on diagnosis and treatment plan in an orthopaedic outpatient shoulder clinic. Furthermore a cost analysis and an evaluation of the interdisciplinary collaboration at the orthopaedic outpatient shoulder clinic (termed shoulder clinic in the following) will be performed.

Shoulder Corticosteroid Injection in Diabetic Patients

In type 2 diabetic patients affected by chronic shoulder pain, subacromial injection with corticosteroid could be an effective treatment. The aim of this study was to measure the risk-benefit ratio of this treatment.Twenty patients with well-controlled diabetes were included in a prospective study. In a first pre-injection phase, patients were asked to measure glycemia for 7 days, before breakfast and dinner, then 2 hours after lunch and dinner. Baseline data including Constant Score (CS), Subjective Shoulder Value (SSV) and Numerical Rating Scale (NRS) for pain were collected. Patients were treated with subacromial injection with 40mg of Methylprednisolone Acetate and 2ml of Lidocaine. At discharge, patients were asked to re-measured glycemia for the following week.

Blood Flow Restriction Training in Patients With Shoulder Pain

Rotator Cuff Tendinosis

This study was planned to investigate the effect of blood flow restriction exercise training on shoulder muscle strength and muscle thickness, and to determine the change in pain and symptoms in patients with rotator cuff tendinopathy. Patients in the study group will perform the rehabilitation exercises with a pneumatic cuff and blood flow restricted. The patients in the control group will perform the same exercise program without restricting blood flow.
